Location: International Falls, Minnesota
Facility: Sanford Home Care and Hospice

Position Details

• Specialty: Physical Therapy (Home Health, Hospice, Long Term Care)
• Work Settings: Home care, hospice, and skilled nursing facilities (SNFs)
• Driving Radius: 40 miles for Home Health visits
• Cases per Day: Approximately 5
• Coverage: Must cover SNFs in International Falls area
• Supervision:

  • Will supervise Physical Therapist Assistants (PTAs)
  • No onsite Therapy Supervisor available
  • No direct supervision for the therapist
• Software Used: Remote Client
• Productivity Expectations: Variable, as position is shared with nursing home
• Scheduling: Monday through Friday, 8:00 AM to 4:30 PM
• Guaranteed Hours: 40 hours per week
• Contract Length: 13 weeks

Requirements & Additional Information

• Experience: Must have home health experience; new graduates may be considered if they had home health experience during clinicals
• Certifications: BLS required
• Mileage & Drive Time: Mileage reimbursed at government rate; drive time is billable
• On-Call: No on-call or call-back required or billable
• Other:

  • No onsite Therapy Supervisor
  • No supervising responsibilities beyond PTAs
  • $500 fee if two weeks' notice is not given before quitting
  • Can start prior to 3 weeks out if cleared
