Job Opportunity: Cath Lab Technician

Location:

• Des Moines, Iowa

Facility:

• MercyOne Des Moines Medical Center

Employment Type:

• Long-Term Assignment (LTA)
• Contract Length: 13 weeks

Schedule:

• 0700 to 1730, four days a week
• Call required: 8 shifts per month

Required Certifications:

• BLS (American Heart Association)
• ACLS

Onboarding and Credential Requirements

Certifications & Licenses

• BLS certification (front and back of card required)
• License/Certificate verification dated within 30 days of start
• Specialty-specific competency testing (exam within the last year)

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen (9 Panel, no THC, completed within 30 days of start)
• COVID-19 Vaccine (including Sept 2024 booster, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (at least first shot prior to start)
• Influenza Vaccine (optional, but recommended; declination accepted except in CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of 1 vaccination or IgG titer)
• Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity Color Vision test

Professional & Compliance Documents

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification, dated within 30 days of start)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) report dated within 30 days
• GSA/OIG (Office of Inspector General) exclusion report dated within 30 days
• Joint Commission CORE Exam within the last year
• NSO (National Sex Offender) check dated within 30 days
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check, within 30 days prior to start)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

This position requires thorough compliance with Trinity Health’s credentialing and onboarding process, including multiple health screenings, background checks, and signed professional agreements.
