Required Certifications
- BLS (Basic Life Support)
- ACLS (Advanced Cardiac Life Support)
- PALS (Pediatric Advanced Life Support)
- NIH Stroke Certification (required for ER and Critical Care RNs)
- NRP (Neonatal Resuscitation Procedure) – required for L&D, Post-Partum, OB
- EFM (Electronic Fetal Monitoring) – facility-specific expiration rules
- State RN License verification within 30 days of start
- NURSYS Verification for RNs
- Orientation Safety Manual Acknowledgement (annual)
- Tech Certification (varies by role, required within 30 days of start)

Competency Exams
- Medication Test/Pharmacology Competency Exam (required prior to start)

- Dysrhythmia/EKG Competency Exam (required within 1 year of start and annually thereafter, with remediation process if failed)
Health & Immunization Requirements
- Physical Exam within 1 year of start confirming good health and free of contagious disease
- Tetanus/Diphtheria/Tdap (adult dose after age 11/12)
- Negative TB test or chest X-ray within 1 year (PPD, QGold, or TSPOT) or TB Questionnaire
- Rubeola (Measles): 2 MMR vaccines or positive titer
- Mumps: 2 MMR vaccines or positive titer
- Rubella: 2 MMR vaccines or positive titer
- Hepatitis B: Titer or 3-vaccine series (declination acceptable)
- Varicella: 2 vaccines or positive titer (medical exemption allowed)
- Negative 10 Panel Drug Screen within 3 months of start (includes amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, methadone, methaqualone, opiates, PCP, propoxyphene)
- Seasonal Flu Vaccination or Declination (required during flu season Oct 1 - Mar 31)
- COVID-19 Vaccine: Not required but strongly recommended
Background & Compliance
- 7-year criminal background check within 90 days of start (includes SS trace, counties lived/worked, nationwide, sex offender, MVR if driving required)
- FBI and BCI fingerprinting required within 1 year of start (code 3701.881)
- Pending background and fingerprint results require attestation
- EPLS/GSA/SAM and HHS/OIG monitoring required monthly
- 2 professional references required (at least one from a manager or formal leadership)
- Employment application and skills checklist required
- Education accreditation confirmation required (accredited nursing program)
- Transcript or diploma verification required for RRTs
Facility Specific Policies
- Dress Code and Hand Hygiene Policy acknowledgment required
- Confidential Fraud, Waste, Abuse, and Compliance Reporting training and form required
- Fraud, Waste, and Abuse Training Certificate required annually
- Ethics and Compliance Policies must be reviewed and signed annually for new contracts
