Job Description

• Position: Home Health Physical Therapist
• Shift: 5 days a week, 8 hours per day (08:00 AM - 04:30 PM)
• Call: Required 1 Sunday every 5 weeks
• Weekend Schedule: Every 5th week, take 2 days off during the week and work Saturday and Sunday to complete admissions
• Coverage Area: 13 counties; travel to furthest areas rotates weekly
• Patient Load: Adjusted based on mileage and travel time

Requirements

• Minimum 1 year of experience in Home Health Physical Therapy
• Required Certifications:

  • BLS (Basic Life Support)
  • DPT (Doctor of Physical Therapy)
  • State License (Missouri)
• Dress Code: No printed scrubs; black, grey, or khaki pants with polo or scrub top

Certifications & Compliance

• BLS: AHA (RQI acceptable) or ARC (must state "Basic Life Support")
• ACLS, PALS, TNCC, NIH Stroke Certification, IV Certification (as applicable for other roles) are noted but not specifically required for this PT role
• Primary Source Verification required for licenses and certifications within 30 days of start
• Annual safety and orientation acknowledgements required
• Pass/Fail testing (e.g., Basic Dysrhythmia exam) applies primarily to nursing staff, not PTs
• Background checks, drug screening, and professional references required

Health & Immunization Requirements

• Physical exam within last 12 months (not required annually)
• Negative TB test or chest X-ray within 1 year, with follow-up protocols for positive results
• Immunizations or documented immunity/declination for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination required during flu season or approved medical/religious exemption (mask required if exempt)

Documentation & Verification

• Background check within 30 days of start, including national and state criminal checks, SSN trace, sex offender registry, and FACIS Level 3
• EPLS/GSA/SAM and HHS/OIG checks required
• Verification of education, work history, and professional references (2 within last 3 years)
• Missouri-specific EDL search required within 30 days of start
• Completion of facility-specific documents such as confidentiality agreement, code of conduct, and orientation quiz
