Job Description

  • Patient assessment and intervention
  • Medication administration
  • Following physician orders and observing patients
  • Documentation in Electronic Health Records (EHR)
  • Implementing and monitoring plan of care
  • Assisting with Activities of Daily Living (ADLs)

Required Credentials & Compliance

Licenses & Certifications

  • Nursys Verification: Must be completed within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions.
  • License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
  • BLS (AHA): Required before onboarding.
  • Driver's License/State ID: Must be current, match address on file, and show front/back.

Exams & Testing (Must be completed prior to start and maintained as required)

  • Ascension Pharmacology Exam (minimum 80%, max 2 attempts)
  • Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, max 2 attempts)
  • Ascension EKG Exam (minimum 80%, max 2 attempts; updated version required)
  • Ascension Skills Checklist (clinical specialty; completed within 1 year and updated annually)

Health & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
  • Fit Test: Required for all travelers; preferred before start, can be done onsite.
  • 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start or negative IGRA within 90 days. TB questionnaire updated annually.
  • Positive PPD: Negative chest X-ray within 1 year and annual TB questionnaire required.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • 12 Panel Drug Screen: Within 30 days prior to start and updated annually; retesting required if break in service > 90 days.

Professional Requirements

  • Resume/Profile: Required for submission.
  • Two Professional References: Required prior to first assignment.
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or if break in service > 90 days.
  • EPLS Check: Within 90 days of start; rerun if break in service > 90 days.
  • Education Verification: Relevant to clinical work; completed within 90 days of start and rerun if break in service > 90 days. Official transcripts required if verification fails.
  • Employment Verification: Past 7 years; updated annually or if break in service > 90 days.
  • Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour training with attestation upload.
  • Nursing Contract Worker Onboarding: Completed onsite.
  • Badge Photo: Forward-facing headshot with white or blue background, no alterations.
  • Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
