Job Description

• Provide physical therapy services in a home health setting with home visits and periodic office meetings.
• Average patients per day: 5-6.
• Patient assignments range from 5-10 per day, equating to roughly 6-8 points per day (30 points expected per week).
• Types of cases: Start of Care (SOC) and Follow-ups.
• Must have reliable transportation and a current driver's license.
• Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
• Must be willing to travel up to 75 miles from the home clinic.
• Weekly productivity expectation: 30 units.
• Work schedule: Monday to Friday, 8 AM to 5 PM.
• Weekend rotation: Not required.
• On-call: Not required.
• Territory: Northern Salem, Oregon.
• EMR system used: HCHB.
• Reason for need: High volume of new referrals pending for PT Start of Care.
• Local candidates accepted at the same rate.
• First-timers not accepted; must have recent home health experience.
• Minimum 1 full year of adult home health experience required.
• CPR and BLS certifications required.
• Pending licenses not accepted unless license is expected within a few days.

Requirements & Qualifications

• Current, valid Physical Therapy license for Oregon.
• Minimum 1 year of recent adult home health experience.
• CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license.
• Agency-required liability insurance.
• Willingness to travel within a 75-mile radius of the home clinic.
• Must be able to use the HCHB EMR system.
• Must meet all background checks, drug screening, and health document requirements (see below).

Compliance & Documentation

Candidates must complete and maintain the following prior to and during the assignment:

Licensure & Background Checks

• Valid PT license in hand.
• Standard background check covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
• EPLS/GSA/SAM and HHS/OIG checks within 14 days of start.
• Fingerprinting as required by state and facility.
• Work history verification (at least 2 most recent employers or last 7 years).
• Education verification for applicable degree.
• References (2 required).

Health & Immunization Requirements

• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• Drug screening (10-panel).
• Seasonal flu vaccination or documented declination.
• COVID-19 vaccination series or documented declination.

• Personal or auto insurance (policy under candidate’s name or candidate listed as driver).
• NDA and End User Agreement.

Work Environment

• Home health setting with patient visits in their homes.
• Periodic office meetings at the clinic located at 3220 State Street Suite 100, Salem, OR 97301.
• Travel required within a 75-mile radius of the home clinic.
• Average weekly travel estimated at 150 miles.
