Experience Requirements
- Minimum 1 year experience in Medical-Surgical (MS) and oncology
- Hematology-Oncology experience strongly preferred
- Chemotherapy certification NOT required

- Telemetry monitoring and dysrhythmia interpretation
- Use of Rapid Response Teams
- Feeding tube care and management
- Blood product administration

- Nasal cannula oxygen administration
Background and Compliance Requirements
- 7-year criminal background check within 90 days of start, including FBI and BCI fingerprinting
- Monthly EPLS/GSA/SAM and HHS/OIG monitoring
- Education accreditation verification from accredited nursing programs (CCNE, ACEN, NLN CNEA, CASN)
- Fingerprinting must be completed using specific Ohio codes; results required prior to start
- Pending background check attestation allowed if fingerprint results are pending
- Drug screening: Negative 10-panel drug screen within 3 months of start (no pending drug screens allowed)
- Physical exam within 1 year of start confirming good health and free of contagious disease
- Immunizations and health documents required:
- Tdap (adult dose)
- Negative TB test or chest X-ray within 1 year
- TB questionnaire
- MMR (Measles, Mumps, Rubella) vaccines or positive titers
- Hepatitis B titer or vaccine series (declination acceptable)
- Varicella vaccines or positive titer (medical exemptions reviewed)
- Seasonal flu vaccine or declination during flu season (mask required if declined)
- COVID-19 vaccination not required but strongly recommended
Competency and Testing Requirements
- Medication/Pharmacology competency exam prior to start
- Unit-specific competency exam
- Dysrhythmia/EKG competency exam required within 1 year of start and updated annually (minimum passing score 76%)
- Remediation process in place for failed EKG exams with up to 3 attempts
Additional Requirements
- 2 professional references required (at least 1 from a manager or formal leadership)
- Employment application including education verification required
- Skills checklist required
- Annual review and acknowledgement of facility policies including dress code, hand hygiene, ethics, and compliance
- Fraud, Waste, and Abuse training certificate required prior to start and updated annually
- Orientation safety manual acknowledgement required annually
