Physical Therapist (PT) Travel Job Opportunity
Location: Racine, Wisconsin
Contract Length: 13 weeks
Job Requirements & Details
- Must hold a current Physical Therapist license.
- Basic Life Support (BLS) certification required.
- Minimum 2 years of acute or inpatient experience.
- Experience with EPIC electronic health record system preferred.
- Weekend work: 6 weekend days during the 13-week contract (already fulfilled).
- Holiday work as needed.
- May be required to float onsite within skill set or to other sites within 40 miles.
Required Credentials & Compliance
- Ascension Specialty Exam: Must be completed prior to first assignment and updated every 3 years. Minimum 80% passing score required. Only 2 attempts allowed.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses of MMR vaccine four weeks apart required.
- Fit Test: Required for all travelers; preferred before start but can be completed onsite. Annual update required.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
- Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest X-ray within one year showing no infection, and completed TB questionnaire. Annual updates required.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- Driver's License/State ID: Must be valid, not expired, and match address on file. For out-of-state placements, address must match home state for compact license.
- 2 Step PPD or IGRA: First PPD within 1 year, second within 30 days of start, or negative IGRA within 90 days of start. Annual TB questionnaire updates required.
- Varicella: Proof of two vaccines, serological immunity, or verified history. No declinations accepted.
- Tdap: Valid within 10 years. No declinations accepted.
- Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually. Must include all 12 drugs tested with lab documentation.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
- Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start. Retesting required if break in service > 90 days.
- Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, and National Sex Offender check. Completed within 30 days of first assignment and updated annually or after breaks in service > 90 days.
- Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.
- Ascension Employment Verification: Third-party verification for past 7 years, updated annually or after breaks in service > 90 days.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension. Must be on agency letterhead with accurate dates.
- Ascension Profile and Professional References: Resume/profile and at least two professional references required prior to assignment.
Additional Notes
- All credentials and documentation must be current and compliant with Ascension Health policies.
- Some credentials are mandatory for onboarding; others are required for submission or ongoing compliance.
- Candidate must be prepared for possible floating to other nearby sites within 40 miles.
- Weekend and holiday coverage required as specified.
