- Shifts: Rotating shifts every 36 hours
Required Credentials and Onboarding Requirements
Certifications
- BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide scanned copy (front and back) with expiration date.
- NRP (Neonatal Resuscitation Program): Required for RNs in Birth Center. Provide scanned copy with expiration date.
Licenses
- License/Certificate Verification: Must be dated within 30 days of start. Online verification page with machine-generated date required. For RNs and LPNs, upload state verification if available (e.g., NURSYS).
Personal Health History
- Fit Test: Required prior to onboarding.
- Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferably within 14 days). Must include specific substances (no THC). Must be completed and resulted before start with MRO signature.
- COVID-19 Vaccine: Proof of vaccine or approved declination/exemption required. Booster valid through September 2025; declination not accepted in CA or IL.
- MMR: Proof of 2 vaccinations with dates or IgG titer lab report required.
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
- Influenza Vaccine: Proof of vaccine, declination, or exemption required for current flu season (Oct 2024 - Apr 2025 and Oct 2025 - Apr 2026). Declination not accepted in CA or IL.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year (certain states) or within 5 years (others).
- Varicella: Proof of 1 vaccination with date or IgG titer lab report required.
- Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
Professional Requirements
- Trinity Code of Conduct Form: Signed form required.
- Trinity I9: Standard I9 completed by agency.
- Confidentiality & Security Agreement: Signed agreement regarding patient and proprietary information required.
- Integrity and Audit Services Form: Signed within days prior to start.
- Conflict of Interest Disclosure Form: Signed form required.
- Criminal Background Check: Federal, nationwide, and county checks covering last 7 years, including SSN verification. Must be dated within 30 days of start.
- Education Verification: Highest level of academic education verified by third party screening report.
- EPLS (Excluded Parties List System): Report dated within past 30 days required.
- GSA/OIG Exclusion Check: Report dated within past 30 days required.
- Joint Commission Testing: CORE exam within last year required.
- NSO (National Sex Offender) Check: Must be dated within 30 days of start, including all aliases and locations. MI requires additional statewide check.
- System Access Form: Must be completed and uploaded one week before start.
Testing and Competency
- Specialty Specific Competency Testing: Must be completed within the last year; attach copy of specialty exam.
- Skills Checklist: Provided by agency for clinical contractors; required for onboarding.
