Position Details

• Shift: 4 days per week, 10 hours per day (4x10)
• Experience Required: Minimum 1 year

• Weekend Requirement: None, unless desired
• On-call Requirement: No
• Floating: May float within scope as needed
• Block Schedule: Schedule requests can be discussed with manager
• Locals Accepted: Yes, no radius restrictions
• Return to Office Restrictions: Ideally no more than 5 days
• Guaranteed Hours: Client may call off up to 36 hours per 13-week assignment without pay; on-call shifts count as called-off shifts

Required Certifications & Licenses

• BLS (Basic Life Support) - AHA (RQI acceptable)
• State License - Primary source verification required
• Additional Certifications: ACLS, PALS may be required based on unit/specialty
• Tech Certification Verification: National primary source verification
• NURSYS Verification: Required for RN, LPN/LVN, AP

Experience & Competency

• Competency exam required for all clinical and allied candidates
• Skills checklist required for clinical and allied candidates
• References: 2 professional references required for clinical/allied candidates

Background & Screening Requirements

• Background check within 30 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
• State-specific searches including Illinois Department of Corrections, Most Wanted, and State Police Murderer and Violent Offender Against Youth Registry within 30 days of start
• Illinois Health Care Worker Registry search required for clinical candidates without IL state license
• Fingerprinting must be completed in Illinois 3 business days prior to start (effective 7/8/2025)
• Education verification required (official or unofficial transcripts accepted if pending)

Health & Immunization Requirements

• Physical exam within 1 year of start, signed by appropriate healthcare provider
• Fit mask test required (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year
• TB questionnaire updated annually
• Immunizations or proof of immunity required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screening: 9-panel drug test within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually by 11/30

Facility-Specific Requirements

• Badge photo required within one week of signing contract
• Code of Conduct must be reviewed and signed within 7 days of contract signing
• Parking permit form must be completed within 7 days of contract signing (unless using alternative transportation)
• Badge name confirmation required
