Job Opportunity: Surgical Technician

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Details

  • Holiday rotation as needed
  • 10-12 days of call per month

Required Credentials & Compliance

Health & Immunizations

  • COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL). Valid through October 2026.
  • MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
  • Fit Test: Required prior to start for Iowa locations including Mason City.
  • Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Hep B titers not accepted.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead. Flu season dates apply (10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026). Declination not accepted for CA or IL.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for some states; otherwise within 5 years.
  • Varicella: Proof of 1 vaccination with date or IgG titer lab report.
  • Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

Drug Screening

  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed. Must have MRO signature.

Professional Requirements

  • Resume and Skills Checklist: Resume, skills checklist within the last year, and 2 professional references required for submission.
  • Confidentiality Agreement: Signed Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement required.
  • Conflict of Interest Disclosure: Signed form required.
  • Criminal Background Check: Federal, nationwide, county criminal check covering last 7 years, including SSN verification and employment verifications. Must be dated within 30 days of start.
  • Education Verification: Highest level of academic education verified by third party screening report. Certifications required if specified.
  • Exclusion Lists: EPLS, GSA/OIG, NSO checks required within 30 days of start.
  • Integrity and Audit Services Form: Signed within days prior to start.
  • Joint Commission Testing: CORE exam within the last year.
  • System Access Form: Must be completed and uploaded one week before start.
  • Child and Adult Dependent Abuse Training: Required for Iowa.
  • Code of Conduct Form: Signed form required.
  • I9 Verification: Standard I9 completed by agency.
  • Iowa-Specific Background Checks: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start.
  • OFAC Sanctions Check: Must be completed within 30 days of start.
  • Specialty Specific Competency Testing: Specialty exam completed within the last year.
  • Non Employee Upload Form: Completed and uploaded pages 1-3 and 28 required.

Certifications & Licenses

  • BLS: American Heart Association certification required for all direct patient care personnel. Must include front and back images of card with expiration date.
  • Certified Surgical Technologist (CST): Required.
  • License/Certificate Verification: NBSTSA Certification required.

Additional Notes

  • Skills checklist will be provided by the agency for clinical contractors.
  • All documentation must be uploaded to the designated system prior to or at onboarding as specified.
