Job Description
- Position: Physical Therapist (Field/Home Health)
- Work Days: Tuesday-Saturday or Wednesday-Sunday
- Home Health experience preferred but candidates without experience will be considered
- California license preferred; candidates with pending CA license who have applied will be accepted
- OASIS experience required
- Guaranteed hours based on completed visits (25 visits = 40 hours)
- Mileage reimbursed at IRS standard rate
- COVID exemptions permitted
Required Credentials & Documentation
Licenses & Certifications
- California Physical Therapy License (preferred or pending with application submitted)
- OASIS experience required
- ACLS (AHA Instructor or Provider) – as applicable
- BLS (AHA Instructor or Provider) – required for all
- PALS, NRP, EFM – as applicable
- National Tech Certification Verification – as applicable
- State License Verification (primary source online)
- NURSYS Verification (for RN, LPN/LVN, AP as applicable)
Background Checks (Annual)
- Standard 7-year background check including SS trace and county searches
- EPLS/GSA/SAM check
- HHS/OIG check
Competency Exams
- Medication Test/Pharmacology Competency Exam (80% passing or meet Angoff score)
- General Competency Exam (80% passing or meet Angoff score)
Health & Immunization Requirements
- Physical exam within 12 months of start
- TDAP vaccine within past 10 years (no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with additional requirements if past positive)
- Proof of immunity or vaccination for Measles, Mumps, Rubella (born 1957 or later)
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella immunity or vaccination (no declinations accepted)
- Negative 9-panel drug screen within 1 year
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID-19 vaccination or approved medical/religious exemption required prior to start
- COVID-19 booster required
Facility Specific Documents
- Government-issued photo ID (Driver’s License or State ID)
- Valid passport copy before start
- Passport-style photo for badge
- Personal or auto insurance with current expiration dates
- Auto registration with current expiration dates
- Signed acknowledgments for training content and hand hygiene policy
