Job Description

• Field position in Home Health setting covering Berkeley/Oakland area
• Work schedule options: Tuesday-Saturday or Wednesday-Sunday
• Local candidates are welcome
• Home Health experience preferred but candidates without experience will be considered
• California license preferred; candidates with pending CA license who have applied may be accepted
• OASIS experience required
• Guaranteed hours based on completing 25 visits (25 visits = 40 hours)
• COVID exemptions permitted
• Mileage reimbursed at IRS standard rates

Required Credentials & Certifications

• California Physical Therapy License (preferred or pending with application submitted)
• OASIS experience
• ACLS (AHA Instructor or Provider) – as applicable
• BLS (AHA Instructor or Provider) – required for all
• PALS, NRP, EFM – as applicable
• National Tech Certification Verification – as applicable
• State License Verification (primary source online verification)
• NURSYS Verification (for RN, LPN/LVN, AP if applicable)

Required Documents & Compliance

• Government issued photo ID (Driver’s License or State ID)
• Valid Passport copy before start
• Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (updated annually)
• Photo for badge (passport style, shoulders up, front facing)
• Personal or Auto Insurance (must be current during assignment)
• Auto Registration (must be current during assignment)
• Physical exam within 12 months of start
• Immunizations and health screenings:

  • TDAP vaccine within past 10 years (no declinations accepted)
  • Negative TB test or chest X-ray within 1 year (with special rules if past positive)
  • Measles, Mumps, Rubella (MMR) proof or titer for candidates born in 1957 or later (no declinations accepted)
  • Hepatitis B titer or vaccine series (declination accepted if within 1 year)
  • Varicella immunity proof or vaccine (no declinations accepted)
  • Negative 9-panel drug screen within 1 year
  • Seasonal flu vaccination/documentation (declinations accepted)
  • COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster

Competency Requirements

• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exam with minimum passing score of 80% or higher if Angoff score applies

Additional Requirements

• Two professional references
• Annual skills checklist
• Completion and acknowledgment of facility-specific training and policies (e.g., hand hygiene)
