    Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

    Location: Glendale, California

    Job Description

    • Position involves providing physical therapy services in a home health setting covering Hesperia, Victorville, and the High Desert area.
    • Home Health experience preferred.
    • California PT license preferred; candidates with pending CA license who have applied may be considered.
    • OASIS experience required; candidates without OASIS experience may be considered if they have solid home health experience.
    • Guaranteed hours based on completed visits: 25 visits equate to 40 hours.
    • COVID exemptions permitted.
    • Mileage reimbursement follows IRS standard rates.

    Required Credentials & Certifications

    • Licenses: California PT license (or pending with application submitted)
    • Certifications:
      • BLS (Basic Life Support) - AHA only, instructor or provider acceptable (required for all)
      • ACLS, PALS, NRP, EFM as applicable (AHA only)
      • State License verification (primary source online verification required)
      • NURSYS verification for RN, LPN/LVN, AP as applicable
      • National Tech Cert Verification as applicable for allied professionals

    Required Documents & Compliance

    • Government-issued photo ID (Driver’s License or State ID)
    • Valid passport copy before start
    • Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (updated annually)
    • Photo for badge (passport style, shoulders up, front facing, plain background)
    • Personal or auto insurance with current expiration dates
    • Auto registration with current expiration dates
    • Facility-specific training and policy acknowledgments (e.g., hand hygiene)
    • Physical exam within 12 months of start
    • Immunizations and health screenings:
      • TDAP vaccine within past 10 years (no declinations accepted)
      • Negative TB test or chest X-ray within 1 year (with special rules for past positives)
      • Proof of immunity or vaccination for Measles, Mumps, Rubella (for those born 1957 or later)
      • Hepatitis B titer or vaccine series (declination accepted if within 1 year)
      • Varicella immunity or vaccination (no declinations accepted)
      • Negative 9-panel drug screen within 1 year
      • Seasonal flu vaccination/documentation (declinations accepted)
      • COVID-19 vaccination or approved medical/religious exemption, including booster (required)

    Competency Requirements

    • Medication Test/Pharmacology Competency Exam (as applicable)
    • Competency Exam with minimum passing score of 80% or higher if Angoff score applies
    • Annual skills checklist

    References

    • Two professional references required
