Job Opportunity: RN OR (Operating Room Nurse)

Location: Ontario, Oregon
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Required Credentials and Onboarding Requirements

Certifications & Licenses

  • BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel. Must provide scanned front and back of card with expiration date.
  • PALS (Pediatric Advanced Life Support)
  • ACLS (Advanced Cardiovascular Life Support)
  • Valid nursing license with verification dated within 30 days of start. State verification preferred (e.g., NURSYS for RNs).

Health & Immunizations

  • Fit Test
  • 10 Panel Drug Screen (completed within 30 days of start, preferred within 14 days; must include specific substances, no THC)
  • COVID-19 Vaccine (proof of booster by Sept 2025, or declination/exemption with agency letterhead; declination not accepted in CA or IL)
  • MMR (Measles, Mumps, Rubella) – proof of 2 vaccinations or IgG titer lab report
  • Hepatitis B – must have started vaccination series prior to start; titers not accepted
  • Influenza Vaccine – proof of vaccine, declination, or exemption (declination not accepted in CA or IL) for current flu season
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) – with chest x-ray if positive TB as per regional requirements
  • Varicella – proof of 1 vaccination or IgG titer lab report
  • Physical/Health Statement – documentation from exam within last year confirming fitness to work

Professional & Compliance Documents

  • Trinity Code of Conduct Form (signed)
  • Trinity I9 form (completed by agency)
  • Confidentiality and Information System Security Agreement (signed)
  • Conflict of Interest Disclosure Form (signed)
  • Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification; dated within 30 days of start)
  • Education Verification (highest academic degree or diploma/GED verified)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG Exclusion check dated within 30 days
  • Integrity and Audit Services Form (signed shortly before start)
  • Joint Commission CORE Exam (completed within last year)
  • NSO (National Sex Offender) check dated within 30 days, including aliases and locations
  • System Access Form (completed and uploaded one week before start)

Testing & Competency

  • Specialty Specific Competency Testing – copy of specialty exam completed within last year
  • Skills Checklist – provided by agency for clinical contractors

This position requires thorough credentialing and compliance documentation prior to onboarding. All certifications, immunizations, and professional verifications must be current and properly documented.
