Job Opportunity: Home Health Physical Therapist (PT)

Job Details

• Shift Options:

  • 5 days a week, 8 hours per day (5x8)
  • 4 days a week, 10 hours per day (4x10)
  • Start time: 8:00 AM
  • Shift preference chosen by traveler

• Call Requirements:

  • Call required 1 Sunday every 5 weeks
  • Every 5th week, work Saturday and Sunday doing admits, with 2 days off during the week

• Experience & Certification Requirements:

  • Minimum 1 year of experience in Home Health Physical Therapy required
  • Required certifications:

    • Basic Life Support (BLS)
    • Doctor of Physical Therapy (DPT) degree
    • Valid State License for Missouri

• Dress Code:

  • Scrubs without prints
  • Acceptable colors: black, grey, or khaki pants with polo or scrub top

• Work Area:

  • Coverage across 13 counties
  • Weekly rotation to furthest areas
  • Patient load adjusted based on mileage and travel time

Background Checks

• National Criminal Check
• State Searches (10 years of all addresses and aliases)
• County Searches (10 years of all addresses and aliases)
• Social Security Number Trace with all aliases
• National Sex Offender Registry
• FACIS Level 3
• EPLS/GSA/SAM
• HHS/OIG
• Education Verification (official transcript or diploma with graduation date)
• Missouri EDL Search (within 30 days of start)

Certifications

• BLS (AHA or ARC; must state "Basic Life Support" if ARC)
• State License Primary Source Verification (within 30 days of start)
• NURSYS Verification (for RN, LPN/LVN, AP as applicable)
• Orientation Safety Manual Acknowledgement (annually)

Health Requirements

• Physical Exam (physician statement within last 12 months)
• Fit Mask Testing (completed onsite at orientation)
• Negative TB Test or Chest X-Ray (within 1 year, updated annually)
• Immunizations or Declinations for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Screening (within 30 days of start, covering multiple substances)
• Seasonal Flu Vaccination or approved medical/religious exemption (required by December 10 annually; mask required if exempt)

Other Requirements

• 2 Professional References within 3 years
• Skills Checklist
• Verification of most recent employer work history
• Facility-specific documents including confidentiality agreement, code of conduct, demographic form, and orientation quiz (must score 84% or higher)

Notes

• Locals are accepted at a reduced rate
• Basic Dysrhythmia exam and other competency exams may be required depending on role and unit (not typically for Home Health PT)
• Mileage and patient load may vary based on travel distance and time
