Job Opportunity: RN ICU - Travel Assignment
Location: Evansville, Indiana

Patient Population & Unit Details
- Patient types: Medical, Neurological, Cardiac, Gastrointestinal, Renal
- Conditions: Sepsis, Trauma, Surgery (excluding fresh open heart surgery)
- Unit size: 24 beds
- Patient ratio: 1:1 to 1:3
Required Certifications & Licenses
- BLS (American Heart Association)
- ACLS (American Heart Association)
- Current nursing license with primary source verification within 30 days prior to assignment
- Nursys verification within 30 days prior to start
- Pharmacology exam (Ascension Pharmacology Exam) with minimum 80% score, updated every 3 years
- Specialty exam (Ascension Specialty Exam) with minimum 80% score, updated every 3 years
- Color Vision test (Ascension Color Vision), updated every 3 years
- Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) with minimum 80% score, updated yearly
- NIH Certification (Group A yearly, Groups B-F every 2 years)
- EKG Exam with minimum 80% score, updated as per latest edition
- Fit Test (annual, preferred prior to start)
- Ascension Skills Checklist (clinical skills), completed within one year of start and updated annually
Health & Immunization Requirements
- MMR: Proof of two vaccines or positive IGG titer; no declinations accepted
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
- TB Screening: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire
- 2 Step PPD or IGRA within 90 days of start
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
- Tdap: Valid within 10 years; no declinations accepted
- Health Statement or physical exam prior to first assignment, updated annually
Background & Verification Requirements
- Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry, and others; updated annually or after break in service > 90 days
- Employment verification for past 7 years; updated annually or after break in service > 90 days
- Education verification relevant to clinical work within 90 days of start; updated if break in service > 90 days
- Professional references: At least two required prior to first assignment
- Drug Screen: 12 panel urine drug screen within 30 days prior to start, updated annually
- Driver’s License/State ID: Must be current and verified
Additional Requirements
- Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
- Ascension Badge Photo (forward-facing headshot with white or blue background)
- Ascension Confirmation of Assignment documentation upon initial assignment and updates with extensions
