Home Health Physical Therapist – LIFE Program

Location: Bordentown, New Jersey (Mercer County & North Burlington County)
Specialty: PACE-LIFE
Contract Length: 13 weeks

Schedule

• Monday – Friday
• 8:00 AM – 4:30 PM
• No on-call duties or weekend work required

About the Role

Provide home health physical therapy services to elderly patients to help maintain their independence. This includes conducting patient evaluations, developing personalized treatment plans, and collaborating with a multidisciplinary care team.

Key Responsibilities

• Conduct patient evaluations and develop individualized treatment plans
• Provide therapy to improve mobility, function, and overall quality of life
• Educate patients and caregivers on exercises and home-based care
• Collaborate closely with physicians, nurses, and case managers
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited Physical Therapy program
• Licensed Physical Therapist in New Jersey
• AHA BLS Certification required
• Minimum 2 years of experience working with adult geriatrics
• Experience in home health and senior care is essential
• Strong communication, documentation, and problem-solving skills
• Reliable transportation required

Preferred Qualifications

• Experience in pelvic floor therapy
• Training in Herman & Wallace Level 1A or equivalent

• Occasional lifting (up to 50 lbs.), bending, and reaching
