Description
Physical Therapist - Travel/Contract
Location: Charleston, West Virginia
Specialty: Therapy/Rehabilitation
Unit: Medical Rehabilitation Unit inside a Trauma hospital (primarily stroke patients ~40%)
Shift: Monday to Friday, 7:30 AM - 4:00 PM (5 days a week)
Patient Ratio: 1 therapist to 5 patients (typically 3-4 patients per day, 90 minutes each)
Contract Length: 13 weeks
Requirements
- Experience: Minimum 1 year required
- First-timers: Accepted
- Certifications: BLS (Basic Life Support) required (AHA only, no pending or onsite completion allowed)
- State License: West Virginia PT license required prior to start; pending state license accepted but must be cleared before starting
- Flu Shot: Required during flu season (Oct 1 - Mar 31) unless medically exempt with documentation; not required outside flu season
- COVID-19 Vaccine: Not required
- On-call: Not required
- Weekend Work: Not required
- Float: Not required
Work Environment & Details
- Scrub Color: Anything except blue, forest green, or maroon (reserved for nursing)
- Charting System: Cerner
- Start Date: Every other Monday once fully cleared
- RTO Restrictions: None
- Special Requirements: None
Documentation & Compliance
Candidates must provide:
- Valid Driver’s License
- Social Security Card (legible)
- Residency Declaration Form (if working on a compact license)
- 7-year Criminal Background Check (within 30 days of start) including SSN trace, county, sex offender, national, and international searches if applicable
- EPLS/GSA/SAM, HHS/OIG, and National Sex Offender Registry checks (within 30 days and updated annually)
- Primary Source Verification of State License (within 30 days of start)
- Skills checklist for specialty (annual completion required)
- Minimum 2 manager/charge nurse references within past 2 years
- Physical exam within 1 year of start, signed by appropriate medical provider, confirming good health and no physical limitations
- Drug screen within 30 days of start, including a comprehensive panel (no placeholders or pending results accepted)
- Annual fit mask testing (N95 3M 1860 Small/Regular or Aura 9205) or PAPR accommodation if needed
- Immunizations and titers:
- Tetanus/Diphtheria/Tdap (required, no declinations accepted; medical/religious exemptions require approval)
- Negative TB test or chest X-ray (within 3 months)
- Rubeola (Measles), Mumps, Rubella (2 vaccines or positive titers required; boosters if negative/equivocal)
- Hepatitis B (series or positive titer required; declination allowed if series completed and negative titer)
- Varicella (2 vaccines or positive titers required; boosters if negative/equivocal)
- Seasonal Flu Vaccination or approved exemption required during flu season (Oct 13 - Apr 1) with strict compliance deadlines
- COVID-19 vaccination is optional; proof or signed declination accepted
Orientation & Training
- Orientation is 2 days (Thursday and Friday) including Cerner Infusion Management
- Pass/fail testing required for nursing only (not applicable for therapy)
- Candidates must complete required inservice training modules prior to start (approx. 4 hours)
- Dress code policy must be reviewed and acknowledged
- Blood administration policy review required for nursing only
- Time and attendance policy review required
- Medical release form and vaccination exemption forms must be completed if applicable
Additional Notes
- Permanent employees cannot return as travelers for 2 years after last permanent day worked at this system
- Color blind testing required prior to start
- CPI (Crisis Prevention Institute) certification required only for certain units (not applicable for PT)
- NBCOT certification required for Occupational Therapy candidates (verify if applicable)
This position offers a structured schedule with no weekend or on-call requirements, working primarily with stroke patients in a rehabilitation setting. Candidates must be fully licensed in West Virginia before starting and comply with comprehensive health and background screening requirements.
