Description
Job Opportunity: RN Labor and Delivery
Location: Silvis, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW) / Every Other Holiday (EOH)
Required Certifications and Licenses
- BLS (Basic Life Support)
- American Heart Association certification required for all direct patient care personnel.
- Must provide scanned copy of certification (front and back) with expiration date.
- ACLS (Advanced Cardiovascular Life Support)
- Required for RNs in critical care areas including Labor and Delivery.
- Must provide scanned copy of certification (front and back) with expiration date.
- NRP (Neonatal Resuscitation Program)
- Required for RNs in Birth Center.
- Must provide scanned copy of certification with expiration date.
- AWHONN/Fetal Heart Monitoring Certification
- Required for Labor and Delivery specialty.
- Valid Nursing License
- License verification must be dated within 30 days of start.
- Online verification with machine-generated date required.
Required Professional Documentation
- Signed Trinity Code of Conduct Form
- Completed Trinity I9 form
- Signed Confidentiality of Patient and Proprietary Information and Information System Security Agreement
- Signed Trinity Integrity and Audit Services Form (signed shortly before start)
- Completed Trinity Non Employee Upload Form (pages 1-3 and 28)
- Completed Trinity Specialty Specific Competency Testing (specialty exam within past year)
- Completed Trinity Profile (resume, skills checklist within a year, 2 professional references)
- Completed Trinity Conflict of Interest Disclosure Form
- Completed Trinity Criminal Background Check (federal, nationwide, county for last 7 years, SSN verification, dated within 30 days of start)
- Completed Trinity Education Verification (highest academic degree or diploma verified)
- Completed Trinity EPLS (Exclusionary List System) check dated within 30 days
- Completed Trinity GSA/OIG (Office of Inspector General) check dated within 30 days
- Completed Trinity IA Sing Report (Iowa-specific criminal and abuse registry check, if applicable)
- Completed Trinity Joint Commission Testing (CORE exam within past year)
- Completed Trinity OFAC (Office of Foreign Assets Control) check within 30 days
- Completed Trinity NSO (National Sex Offender) check dated within 30 days
- Completed Trinity System Access Form (must be completed and uploaded one week before start)
- Completed Trinity Skills Checklist (provided by agency for clinical contractors)
Required Personal Health Documentation
- Fit Test
- COVID-19 Vaccine
- Proof of booster by September 2025 required.
- Declination not accepted in Illinois.
- MMR (Measles, Mumps, Rubella)
- Proof of 2 vaccinations or IgG titer lab report required.
- Hepatitis B Vaccination Series
- Must have started series prior to start date. Titers not accepted.
- Influenza Vaccine
- Proof of vaccine or exemption required for 2024-2025 season.
- Declination not accepted in Illinois.
- 2 Step PPD OR IGRA (Quantiferon/Tspot)
- TB testing required; positive TB requires chest x-ray within last year or 5 years depending on region.
- Varicella (Chickenpox)
- Proof of vaccination or IgG titer lab report required.
- Tdap (Tetanus, Diphtheria, Pertussis)
- Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
- Physical/Health Statement
- Documentation from exam within last year confirming fitness to work.
- Trinity 10 Panel Drug Screen
- Completed within 30 days of start, must include specific substances; no THC allowed.
This position is ideal for experienced RNs with Labor and Delivery specialty who meet the above certification, licensing, and health requirements and are available for a 13-week assignment in Illinois.
