Surgical Technician - Mason City, Iowa (LTA Contract)

Job Details

  • Specialty: Surgical Technician
  • Location: Mason City, Iowa
  • Employment Type: Long-Term Assignment (LTA)
  • Shift: Mostly day shift, 8 hours (6:30 AM - 3:00 PM, depending on department needs)
  • Call Requirement: 10-12 days per month
  • Weekend Requirement: Every third weekend
  • Contract Length: 13 weeks

Required Credentials & Compliance

Health & Immunizations

  • COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL).
  • MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
  • Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption with agency letterhead (declination not accepted for CA or IL). Valid for flu season 10/1/2024 - 4/30/2025.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within last year (for some states) or within 5 years for others.
  • Varicella: Proof of 1 vaccination with date or IgG titer lab report.
  • Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
  • Fit Test: Required prior to start for Iowa locations (Des Moines, West Des Moines, Newton, Waterloo, Siouxland).
  • Trinity Color Vision: Required.

Drug Screening

  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferably within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone. No THC allowed. Must have MRO signature.

Professional Requirements

  • Trinity Profile: Resume, skills checklist within a year, and 2 professional references required for submission.
  • Confidentiality & Security Agreement: Signed Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement required.
  • Conflict of Interest Disclosure Form: Signed form required.
  • Criminal Background Check: Federal, nationwide, county criminal check for last 7 years, including SSN verification and employment verifications. Must be dated within 30 days of start.
  • Education Verification: Highest academic education verified by 3rd party screening report. Certifications required if specified.
  • EPLS (Excluded Parties List System): Report dated within past 30 days required.
  • GSA/OIG Exclusion Check: Report dated within past 30 days required.
  • Integrity and Audit Services Form: Signed within days prior to start.
  • Joint Commission Testing: CORE exam within the last year required.
  • NSO (National Sex Offender) Check: Dated within 30 days of start, all aliases searched.
  • System Access Form: Must be completed and uploaded one week before start.
  • Skills Checklist: Provided by agency for clinical contractors.
  • Child and Adult Dependent Abuse Training: Required for Iowa assignments.
  • Code of Conduct Form: Signed form required.
  • I9 Verification: Completed by agency.
  • Iowa Sing Report: Criminal history, sex offender, dependent adult & child abuse registries check required within 30 days prior to start.
  • OFAC Sanctions Check: Must be completed within 30 days of start.
  • Specialty Specific Competency Testing: Specialty exam completed within a year required.
  • Non Employee Upload Form: Completed and uploaded pages 1-3 and 28 required.

Certifications & Licenses

  • BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must include front and back images of card.
  • Certified Surgical Technologist (CST): Required.
  • License/Certificate Verification: NBSTSA Certification required.

This position requires thorough compliance with health, professional, and credentialing standards prior to and during the assignment.
