Job Opportunity: Physical Therapist (Travel/Contract)
Location: Mullins, South Carolina

Position Details
- Shift: 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
- Weekend Requirement: No
- On-call Requirement: No
- Holiday Requirement: Possibly
- Floating: May be required to float within scope to meet facility needs or between like units/sister facilities
- Experience Required: Minimum 1 year
- First-time Travelers: Not accepted
- Licensure Required: South Carolina or Multi-State Compact Physical Therapist License at contract start
- Certifications Required: Basic Life Support (BLS)

Important Notes
- Hurricane Season: June through November; employees and contractors may be assigned to emergency response teams during this time. Confirm availability before accepting assignments during peak months (August and September).

Required Documentation & Compliance
Licensure & Certifications
- Valid SC or Multi-State Compact Physical Therapist License (primary source verification required)
- BLS certification (AHA or ARC only), must be valid beyond 30 days from start date
- Other certifications (if applicable) must also be current and valid beyond 30 days from start date
Background Checks
- Annual background check covering all locations lived/worked in the past 7 years
- Includes SSN trace, statewide and national criminal checks, sex offender registry, FACIS 3, and international searches if applicable
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
Health & Immunization Requirements
- Initial physical exam, updated annually, confirming ability to meet physical demands
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination form
- Negative TB test (T-spot or Q-Gold within 90 days) or chest X-ray if positive history
- Annual TB questionnaire
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, vaccine series, or declination)
- Varicella
- 10-panel drug screen within 90 days of start (no instant tests)
- Seasonal flu vaccination or medical/religious exemption required during flu season (Aug 1 - Apr 30)
- COVID-19 vaccination highly preferred but not required; declination form required if not vaccinated
Facility-Specific Requirements
- Completion of orientation modules prior to start (approx. 2 hours)
- Upload COVID and flu vaccine documentation into facility system
- Clear, passport-style photo for badge
- Signed assignment acknowledgment, confidentiality agreements, code of conduct, and other HR policy documents prior to start
- Proof of 2 clinical references and recent performance evaluation (within 12 months)
- Verification of last 3 clinical employers (or as many as applicable)
- Radiation badge may be required depending on unit (not typical for Physical Therapists)
Additional Notes
- Travelers must comply with all facility-specific onboarding and compliance requirements, including timely submission of all required documents and certifications.
- Failure to complete required modules or submit documentation may result in delayed start or removal from assignment.
This position is ideal for experienced physical therapists licensed in South Carolina or holding a multi-state compact license, who are comfortable working in a hospital-based rehab setting with both inpatient and outpatient responsibilities.
