Job Description

• Schedule: 4x10 hour shifts, Monday through Friday
• Call Requirements: Minimum one weekday call shift per week and at least one weekend day per month
• Workload: Running approximately 5-6 operating rooms per day

Required Certifications & Licenses

• BLS (Basic Life Support) – American Heart Association only
• ACLS (Advanced Cardiovascular Life Support) – American Heart Association only
• Permanent RN License – Primary source verification required within 30 days of start; temporary licenses not accepted
• MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years or approved booster protocol
• Varicella – Positive titers within last 15 years or approved booster protocol
• Hepatitis B – Proof of full series, partial series, or signed declination (agency form accepted)
• Tdap – Vaccination within last 10 years; no declinations accepted
• Influenza Vaccine – Required during flu season (Oct-Mar) or completion of specific declination/medical exemption forms
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Negative results within last 6 months; if positive, clear chest x-ray and current TB questionnaire required
• Fit Test – Completed within 1 year prior to start (specific mask types required)

Required Professional Verifications & Checks

• Ohio Fingerprints – BCI&I check and FCRA-compliant national criminal background check (requirements vary based on length of Ohio residency)
• Criminal Background Check – Within 6 months of start, including social security trace, county searches, national sex offender registry
• Education Verification – Highest level verified through third-party; international education requires equivalency documentation
• Employment Verification – Verification of employment within last 5 years through third-party
• Social Security Trace – If not included in background check, to be completed within 1 year of start
• National OIG Exclusion Search – To be completed within 6 months prior to start
• SAM/EPLS/GSA/OFAC Screening – To be completed within 6 months prior to start
• Drug Screen – 10-panel urine drug screen within 60 days of start; instant drug screens not accepted
• I-9 Attestation – Proof of eligibility to work in the U.S.

Required Competency & Training

• UHS Competency Exam – Must score at least 80%, completed within 1 year prior to start
• UHS Core Competencies – Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required, completed within 1 year prior to start
• Physical/Health Statement – Pre-employment physical exam within 1 year, stating ability to work without restrictions
• UHS Module Completion Certificate – Completion of required safety and compliance training modules prior to clearance; annual renewal required

Additional Requirements

• UHS Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only)
• Driver's License/State ID – Non-expired, required for submission
• UHS Badge Photo – Professional headshot (shoulders up, plain background, JPEG format)
• Submission Packet – Resume, 2 professional references (within last 2 years), job application, and UH employment history form

• Temporary licenses are not accepted; permanent RN license required
• Attestations are generally not accepted for verifications and must be supported by official documentation
• Some vaccinations and titers require boosters and follow-up testing as per protocol
• Compliance with all credentialing and onboarding requirements is mandatory for assignment start
