Description
Job Opportunity: Cath Lab Technician
Location: Athens, Georgia
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Required Certifications & Licenses
- BLS (Basic Life Support)
- American Heart Association certification required for all direct patient care personnel.
- Must provide scanned copy of certification (front and back) with expiration date.
- ACLS (Advanced Cardiovascular Life Support)
- Required for submission.
- License/Certificate Verification
- Must be dated within 30 days of start.
- Attach scanned online verification with machine-generated date.
- For RNs and LPNs, upload state verification (e.g., NURSYS).
- California requires a picture of the hard copy license for RNs.
Health & Immunization Requirements
- COVID-19 Vaccine
- Proof of September 2025 booster, declination, or exemption (with agency letterhead).
- Declination not accepted for CA or IL.
- MMR (Measles, Mumps, Rubella)
- Proof of 2 vaccinations with dates or primary source IgG titer with lab printout.
- Hepatitis B
- Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later).
- Hep B titers not accepted.
- Influenza Vaccine
- Proof of vaccine, declination, or exemption (with agency letterhead).
- Declination not accepted for CA or IL.
- Flu season dates: 10/1/2024 - 4/30/2025 and 10/1/2025 - 4/30/2026.
- 2 Step PPD OR IGRA (Quantiferon/Tspot)
- Required.
- Positive TB requires chest x-ray within last year for MI, IN, CT, MA; within 5 years for other regions.
- Varicella
- Proof of 1 vaccination with date or primary source IgG titer with lab printout.
- Physical/Health Statement
- Documentation from an exam within the last year confirming fitness to work.
- Fit Test
- Required.
- Trinity Respirator Evaluation
- Physician signature required on provided form.
Drug Screening
- Trinity 10 Panel Drug Screen
- Completed within 30 days of start (preferably within 14 days).
- Must be completed and resulted before start with MRO signature.
- Panel includes: Cocaine, Amphetamines, Opiates, 6-Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, Methadone.
- NO THC allowed.

- Resume, Skills Checklist within a year, 2 Professional References required for submission.
- Confidentiality & Compliance Forms
- Confidentiality of Patient and Proprietary Information and Information System Security Agreement (signed).
- Conflict of Interest Disclosure Form (signed).
- Code of Conduct Form (signed).
- Integrity and Audit Services Form (signed shortly before start).
- Background Checks & Verifications
- Criminal Background Check: Federal, Nationwide, County (last 7 years), SSN verification, employment verifications (last 7 years). Must be dated within 30 days of start.
- Education Verification: Highest academic degree or diploma/GED verified by 3rd party screening.
- EPLS (Excluded Parties List System) report dated within past 30 days.
- GSA/OIG Exclusion Check dated within past 30 days.
- NSO (National Sex Offender) check dated within 30 days, including all aliases and locations.
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days.
- System Access Form
- Must be completed and uploaded one week before start.
- Trinity Joint Commission Testing
- CORE Exam within the last year.
- Trinity Skills Checklist
- Provided by agency for clinical contractors.
- I9 Form
- Standard I9 completed by the agency.
