    Job Description

    Seeking a full-time Physical Therapist in a hospital setting with a focus on Outpatient Rehab and Inpatient coverage.

    Required Certifications

    • California Physical Therapist License
    • Basic Life Support (BLS) Certification

    Job Duties

    • Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions from disease or injury.
    • Gain patient and family cooperation through competent assessment, monitoring, care, and communication.
    • Adhere to all local, state, and federal regulations to ensure privacy and safety.
    • Provide clinical supervision of assistants and aides; serve as clinical instructor for assigned students.

    Rehabilitation Services Responsibilities

    • Plan and implement individualized physical treatment programs to maintain, improve, or restore physical function and alleviate pain.
    • Complete and document patient evaluations and tests per standards and regulations.
    • Develop and update patient Plans of Care based on assessments and diagnoses.
    • Provide and progress treatment per Plan of Care.
    • Complete all documentation related to charge capture.
    • Coordinate patient care with other providers and support team members.
    • Work flexible shifts (days of week, hours).
    • Provide clinical supervision and instruction to assistants, aides, and students.

    Clerical Duties

    • Prepare and complete accurate documentation of all therapy performed in medical and departmental records.
    • May perform general clerical tasks such as answering phones, updating appointments, and managing electronic health records (EHR).

    Safety and Compliance

    • Participate in quality improvement and interdisciplinary projects.
    • Maintain a clean, safe working environment; clean and disinfect equipment after use.
    • Inspect and use equipment per manufacturer guidelines; report malfunctions.
    • Maintain strict confidentiality of all patient Protected Health Information (PHI).
    • Prepare, administer, and document services in compliance with state and federal regulations.
    • Retain certifications and complete mandated continuing education to meet accreditation and licensure requirements.

    Education

    • Graduate of an accredited physical therapy graduate program

    Background and Compliance Requirements

    • 7-year background check required within 6 months of start, including social security trace, county searches, sex offender search, education verification, and other screenings.
    • Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS Level 3 screening.
    • Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
    • TB screening: Negative TB test or chest x-ray within 90 days of start; annual renewal required.
    • Immunizations: Must meet requirements for Tetanus/Diphtheria/Tdap, Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination during flu season. Religious and medical exemptions considered with proper documentation.
    • Physical exam required within 1 year of start.
    • Fit mask test required within 1 year of start; must be renewed annually.
    • Respiratory safety clearance required prior to start; renewed annually.

    Certifications and Competencies

    • BLS (Basic Life Support) required.
    • ACLS, PALS, NRP, CCRN, TNCC, EFM, CPI, NIH Stroke Certification, and other certifications may be required depending on position specifics.
    • State license verification and professional license copies required.
    • Competency exams (medication test, dysrhythmia/EKG, etc.) required as applicable by profession.

    Documentation and Orientation

    • Candidates must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and regulatory orientation prior to start.
    • TB questionnaire must be completed and signed by candidate and healthcare provider within 90 days of start.
    • Employment application must include full 7 years of work history with no gaps.
    • References required per Aya Healthcare standards (not required by client).

    Additional Notes

    • Flexible shift assignments may be required.
    • Clinical supervision and student instruction are part of the role.
    • Compliance with all facility policies and procedures is mandatory.
    • Religious and medical exemptions for immunizations and fit testing may be considered with proper documentation and approval.

    This position offers an opportunity to work in a hospital environment focusing on outpatient rehabilitation with inpatient coverage, requiring strong clinical skills, regulatory compliance, and the ability to supervise and instruct others.
