Therapy PT Home Health Travel/Contract Position
Location: Vidalia, Georgia
Specialty: Therapy/Rehabilitation (Physical Therapy)

Contract Length: 13 weeks
Job Description
- Provide physical therapy services in home health settings, including patient home visits and assisted living facilities (ALFs).
- Periodic office meetings required.
- Average patients per day: 5-6 (assigned 5-10 patients daily, equating to roughly 6-8 points per day).
- Types of cases: Start of Care, Follow Ups.
- Schedule: Monday to Friday, 8 AM to 5 PM (most assignments are standard 5x8s; some may have 4x10 shifts Friday-Monday).
- No guaranteed hours; if patient census is low, floating to nearby clinics may be arranged.
- Travel: Must be willing to travel up to 75 miles from home clinic; approximately 30-40 miles per day, up to 200 miles weekly. Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
- EMR Systems used: HomeCare HomeBase, Oasis, Outlook, Teams messaging.
- On-call: No on-call required for this position.
- Local candidates accepted at the same rate.
Requirements
- Minimum 1 full year of recent adult home health experience required.
- First-time home health therapists not accepted.
- Certifications required: CPR and BLS (Basic Life Support) current.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Must have license in hand before starting (pending licenses accepted only if license issuance is expected within a few days).
- Willingness to discuss weekend and on-call requirements during interview as they may vary by location.
Productivity Expectations
- Weekly productivity units: 25-30 points.
- Points system based on types of visits and activities (documentation, travel, etc.).
Compliance and Documentation Requirements
- Valid Driver’s License/State ID required.
- Background checks including 7-year history, SS trace, county, nationwide, sex offender, and state-specific searches as applicable.
- Drug screening: 10-panel drug test required.
- Health documents:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella as per standards.
- Seasonal flu and COVID-19 vaccination documentation or declination required.
- Education verification and work history verification (at least 2 most recent employers or last 7 years).
- References: Two references required for all placements.
- Facility-specific compliance documents including confidentiality forms, HIPAA and privacy policies, compliance training, and various state-specific forms must be reviewed and signed prior to start.
- Candidates must complete required policy reviews and quizzes prior to assignment start.
Additional Notes
- Candidates must sign new supplier packets at the start of a new assignment and annually if working more than one consecutive year.
- Some state-specific requirements apply depending on placement location (e.g., fingerprinting, registry checks).
- Candidates should be prepared to complete all required documentation and background checks timely to avoid delays in start date.
