Registered Nurse Travel/Contract Position
Location: Mayfield Heights, Ohio
Contract Length: 10 weeks

Requirements & Documentation
Identification
- Government-issued photo ID (Driver's License or State ID) required and must be carried during assignment.
Background Checks
- Comprehensive background check required prior to start, including:
- FCRA compliant 7-year search (SSN Trace, Counties, States, Nationwide, National Sex Offender, OFAC, FACIS Level 3, Education Verification).
- Must be updated annually and after breaks in service of 2+ months.
- Monthly EPLS/GSA/SAM and HHS/OIG checks required.
- FBI and BCI fingerprinting required within 12 months of start, updated annually.
- Fingerprints must be completed under code 4723.09 (Nursing).
- Facility will conduct their own fingerprinting in addition to agency process.
- Candidates may start with fingerprint results pending if fingerprinting is completed prior to start.
- Education verification required for all candidates. International education requires CGFNS documentation as placeholder if needed.
Certifications
- ACLS (Advanced Cardiac Life Support): AHA Instructor or Provider or RQI acceptable. Required for many clinical areas (e.g., ICU, Med/Surg, ER, Pediatrics). Must be current at start.
- BLS (Basic Life Support): AHA Instructor or Provider or RQI acceptable. Required for all candidates. Must be current at start.
- PALS (Pediatric Advanced Life Support): Required for PICU, Pediatric Stepdown, ER Pediatric OR preferred but not required for PACU. Must be current at start.
- NRP (Neonatal Resuscitation Program): Required for Labor & Delivery, OB/GYN, NICU. Must be current at start.
- CHEMO (Chemotherapy Certification): Not required for Oncology.
- TNCC (Trauma Nursing Core Course): Preferred but not required for ER.
- Tech Cert Verification (National): Required for CVOR and NBSTSA certifications.
- Orientation Safety Manual Acknowledgement: Required annually.
- Acknowledgement of Pass/Fail Testing: Required for all nursing positions except OR. Not required for legacy candidates with less than 6-month gap between contracts.
- State License Verification: Primary source verification within 30 days of start for new and extension contracts. License must be current.
- NURSYS Verification: Required within 30 days of start for RN, LPN/LVN, AP.
- NIH Stroke Certification: Preferred for many units; must be current and renewed as required.
- Pass/Fail Testing (EKG/Medication): Required for bedside RNs (except OR). Testing done onsite during orientation. Retake available after 1 week if needed. Not required for legacy candidates with less than 6-month gap.
Competencies
- Medication Test/Pharmacology Competency Exam required as applicable.
- Unit-specific competency exam required within 12 months of start and updated annually.
Facility-Specific Requirements
Online Training Modules
- Must complete all required online onboarding and training modules prior to clearance to start.
- Modules must be completed on a computer using Google Chrome; cannot be completed on a phone.
- Training videos must be watched before taking tests; skipping videos locks candidate out.
- Modules typically take multiple days to complete and are high priority.
- Candidates returning to the system with less than 6-month gap may not need to repeat modules.
SafeMedicate Prep Materials
- Candidates should review materials in preparation for pass/fail exam during orientation. Not required for legacy candidates with less than 6-month gap.
Health & Immunization Requirements
- Physical Exam: Required within 1 year of start, including assessment of ability to perform essential job functions. Snellen eye exam included for exams signed after 4/11/25.
- Fit Mask: N95 fit test required within 12 months of start and updated annually. PAPRs may be approved on a case-by-case basis for exemptions.
- Tetanus/Diphtheria/Tdap: Vaccine required every 10 years; religious/medical exemptions reviewed case-by-case (declinations not accepted).
- TB Testing: True 2-step PPD or QGold/TSPOT within 12 months of start, updated annually. Chest X-ray required if past positive TB test. TB questionnaire required annually if history of positive.
- MMR (Measles, Mumps, Rubella): 2 MMR vaccines or positive titer required. Booster required if titers negative/equivocal. Exemptions reviewed case-by-case (declinations not accepted).
- Hepatitis B: Titer, series, or declination documentation required.
- Varicella: 2 vaccines or positive titer required. Booster required if titers negative/equivocal. Exemptions reviewed case-by-case (declinations not accepted).
- Drug Testing: Standard 10-panel drug screen within 12 months of start, updated annually. Instant drug screens not accepted.
- Flu Vaccine: Required during flu season (10/31 - 03/31) or approved medical/religious exemption. Declinations not accepted.
- Eye Exam: Required within 1 year of start for new candidates signing 4/11/25 or later. Not required for return candidates with less than 40-day gap.
Employment History & References
- Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care and similar area to assignment.
- Two professional references required from employment within 1 year of start, from Manager, ANM, Supervisor, Charge Nurse, or Director (co-worker references not accepted).
- Skills checklist required annually.
State-Specific Requirements (Ohio)
