Registered Nurse Travel/Contract Position

Location: Mayfield Heights, Ohio
Contract Length: 10 weeks

Requirements & Documentation

Identification

• Government-issued photo ID (Driver's License or State ID) required and must be carried during assignment.

Background Checks

• Comprehensive background check required prior to start, including:

  • FCRA compliant 7-year search (SSN Trace, Counties, States, Nationwide, National Sex Offender, OFAC, FACIS Level 3, Education Verification).
  • Must be updated annually and after breaks in service of 2+ months.
• Monthly EPLS/GSA/SAM and HHS/OIG checks required.
• FBI and BCI fingerprinting required within 12 months of start, updated annually.

  • Fingerprints must be completed under code 4723.09 (Nursing).
  • Facility will conduct their own fingerprinting in addition to agency process.
  • Candidates may start with fingerprint results pending if fingerprinting is completed prior to start.
• Education verification required for all candidates. International education requires CGFNS documentation as placeholder if needed.

Certifications

• ACLS (Advanced Cardiac Life Support): AHA Instructor or Provider or RQI acceptable. Required for many clinical areas (e.g., ICU, Med/Surg, ER, Pediatrics). Must be current at start.
• BLS (Basic Life Support): AHA Instructor or Provider or RQI acceptable. Required for all candidates. Must be current at start.
• PALS (Pediatric Advanced Life Support): Required for PICU, Pediatric Stepdown, ER Pediatric OR preferred but not required for PACU. Must be current at start.
• NRP (Neonatal Resuscitation Program): Required for Labor & Delivery, OB/GYN, NICU. Must be current at start.
• CHEMO (Chemotherapy Certification): Not required for Oncology.
• TNCC (Trauma Nursing Core Course): Preferred but not required for ER.
• Tech Cert Verification (National): Required for CVOR and NBSTSA certifications.
• Orientation Safety Manual Acknowledgement: Required annually.
• Acknowledgement of Pass/Fail Testing: Required for all nursing positions except OR. Not required for legacy candidates with less than 6-month gap between contracts.
• State License Verification: Primary source verification within 30 days of start for new and extension contracts. License must be current.
• NURSYS Verification: Required within 30 days of start for RN, LPN/LVN, AP.
• NIH Stroke Certification: Preferred for many units; must be current and renewed as required.
• Pass/Fail Testing (EKG/Medication): Required for bedside RNs (except OR). Testing done onsite during orientation. Retake available after 1 week if needed. Not required for legacy candidates with less than 6-month gap.

Competencies

• Medication Test/Pharmacology Competency Exam required as applicable.
• Unit-specific competency exam required within 12 months of start and updated annually.

Facility-Specific Requirements

Online Training Modules

• Must complete all required online onboarding and training modules prior to clearance to start.
• Modules must be completed on a computer using Google Chrome; cannot be completed on a phone.
• Training videos must be watched before taking tests; skipping videos locks candidate out.
• Modules typically take multiple days to complete and are high priority.
• Candidates returning to the system with less than 6-month gap may not need to repeat modules.

SafeMedicate Prep Materials

• Candidates should review materials in preparation for pass/fail exam during orientation. Not required for legacy candidates with less than 6-month gap.

Health & Immunization Requirements

• Physical Exam: Required within 1 year of start, including assessment of ability to perform essential job functions. Snellen eye exam included for exams signed after 4/11/25.
• Fit Mask: N95 fit test required within 12 months of start and updated annually. PAPRs may be approved on a case-by-case basis for exemptions.
• Tetanus/Diphtheria/Tdap: Vaccine required every 10 years; religious/medical exemptions reviewed case-by-case (declinations not accepted).
• TB Testing: True 2-step PPD or QGold/TSPOT within 12 months of start, updated annually. Chest X-ray required if past positive TB test. TB questionnaire required annually if history of positive.
• MMR (Measles, Mumps, Rubella): 2 MMR vaccines or positive titer required. Booster required if titers negative/equivocal. Exemptions reviewed case-by-case (declinations not accepted).
• Hepatitis B: Titer, series, or declination documentation required.
• Varicella: 2 vaccines or positive titer required. Booster required if titers negative/equivocal. Exemptions reviewed case-by-case (declinations not accepted).
• Drug Testing: Standard 10-panel drug screen within 12 months of start, updated annually. Instant drug screens not accepted.
• Flu Vaccine: Required during flu season (10/31 - 03/31) or approved medical/religious exemption. Declinations not accepted.
• Eye Exam: Required within 1 year of start for new candidates signing 4/11/25 or later. Not required for return candidates with less than 40-day gap.

Employment History & References

• Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care and similar area to assignment.
• Two professional references required from employment within 1 year of start, from Manager, ANM, Supervisor, Charge Nurse, or Director (co-worker references not accepted).
• Skills checklist required annually.

State-Specific Requirements (Ohio)

• BCI and FBI fingerprint results required within 12 months of start and updated annually.
• Fingerprints must be completed under nursing code 4723.09.
