Physical Therapist (Outpatient) - Travel/Contract Position

Location: Cades, South Carolina
Specialty: Therapy/Rehabilitation


Position Details

  • Shift: Days, Monday through Thursday 8:00 AM to 5:00 PM, Friday 8:00 AM to 12:00 PM
  • Patient Ratio: 1:1 (45 minutes per session)
  • Weekends: Not required
  • On-call: No
  • Floating: No floating between units or sister facilities
  • Contract Length: 13 weeks

Requirements

  • Experience: Minimum 2+ years
  • License: South Carolina Physical Therapist License required
  • Certifications: Basic Life Support (BLS) required (AHA or ARC only)
  • First-time travelers: Accepted
  • Local Radius: 60 miles (locals not accepted)

Documentation & Compliance Requirements

Licensing & Certifications

  • South Carolina PT license primary source verification required
  • BLS certification (AHA or ARC only), must be valid beyond 30 days from start date
  • Other certifications (if applicable): ACLS, PALS, NRP, TNCC, EFM, NIH Stroke, IV Certification (for select LPNs), Tech Cert Verification (national certifications for allied professionals)
  • NURSYS verification required for applicable licenses
  • Radiation badge required only if assigned to OR, Cath Lab, EP Lab, or Radiology (not required for Ultrasound Techs)

Background Checks

  • Annual background check covering all locations lived/worked in past 7 years
  • Includes SSN trace, statewide, national criminal, sex offender registry, FACIS 3, and international searches
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for renewals
  • Education verification not required for this facility

Health & Immunization

  • Initial physical exam, updated annually, attesting ability to meet physical demands
  • Tetanus/Diphtheria/Tdap vaccine within 10 years or declination form required
  • Negative TB test or chest X-ray within 90 days of start (2-step PPD or Quantiferon Gold not accepted; T-spot or Q-Gold accepted)
  • Annual TB questionnaire required
  • Measles (Rubeola), Mumps, Rubella: Positive titer or 2 vaccines required; low titer requires boosters
  • Hepatitis B: Titer, vaccine series, or declination required
  • Varicella: Positive titer or 2 vaccines required; low titer requires boosters
  • 10-panel drug screen within 90 days of start (no instant tests)
  • Seasonal flu vaccine required during flu season (Aug 1 - Apr 30), medical/religious exemptions accepted but no declinations
  • COVID-19 vaccination not required but highly preferred; vaccination records must be uploaded or declination form completed
  • Respirator medical clearance required annually for patient-facing roles

Facility Specific Documents & Training

  • First Day Instructions and OurDay modules must be completed prior to start (approx. 2-6 hours depending on campus)
  • Passport-style photo for badge required
  • Signed assignment acknowledgment, code of conduct, employee confidentiality agreement, HR policy manual, systems user agreement, user access request, job description, HIPAA confidentiality agreement required prior to start
  • Orientation modules must be completed before orientation; failure to complete results in being sent home
  • Proof of 2 clinical references required
  • Performance evaluation dated within 12 months of start required
  • Complete employment application/resume required
  • Skills checklist updated annually required for applicable units

  • Facility may call off 1 shift per 2 weeks
  • All required documents and certifications must be valid and uploaded prior to start
  • Candidates with certifications expiring within 30 days of start must renew or have a course scheduled before starting
