Required Credentials & Compliance for Onboarding
Certifications
- BLS (Basic Life Support): Required for onboarding. Acceptable certifications include AHA, Red Cross, CHSF, or Military.
- ARRT (American Registry of Radiologic Technologists): Required for onboarding.
Licenses
- Professional License/Certificate Verification: Must be verified through primary source within 30 days of start date and updated upon expiration. MA Conditional Licenses are not accepted.
Personal Health Requirements
- Fit Test (N95 Respirator): Required within 365 days of start date and updated annually. Must be completed on the specific MGB N95 Respirator Fit Test Record Form. No religious exemptions.
- MMR Vaccine or Immunity Proof: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
- Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed. Pharmacy or provider administered flu mist accepted; self-administered not accepted.
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date, updated if break >6 months.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date, updated if break >6 months.
- Respirator Medical Clearance (RMC) Attestation: Required if fit test is required; must be completed prior to fit testing.
Optional but Preferred Health Credentials
- Hepatitis B: Strongly preferred; record of 3 vaccines or proof of immunity or declination.
- Tdap Vaccine: Preferred within last 10 years and updated upon expiration.
- Positive PPD - Negative Chest X-Ray: Required if positive TB test.
- TB Symptom Survey: Recommended if positive PPD; must be completed prior to start and updated annually.
Professional Requirements
- Criminal Background Check: Must be completed within 45 days prior to start and updated if break >6 months. Includes county and international searches as applicable.
- Massachusetts CORI Check: Must be run within 30 days prior to start and updated if break >6 months.
- National Sex Offender Check: Run within 30 days of start and updated annually.
- Exclusion Checks: Run within 30 days of start and annually; includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists.
- Social Security Number Trace: Must be run with criminal check covering last 7 years.
- Education Verification: Verified through background check company; must verify highest level of education achieved.
- References: Two recent references required, at least one supervisor or above in skillset.
- Resume: Must include past 7 years of work experience with city and state.
- Skills Checklist: Required for submission.
- Candidate Submission Coversheet: Required for submission.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions, updated if start date changes.
