Physical Therapist Outpatient - Orleans, Massachusetts

Job Summary

• Assist patients in reaching maximum physical performance while adjusting to disabilities.
• Restore patient physical functioning by interpreting physical therapy evaluations and test results.
• Determine physical therapy treatment goals and plans.
• Administer manual exercises.
• Instruct, encourage, and assist patients in performing physical activities and using assistive/supportive devices.

Required Credentials and Compliance for Onboarding

Certifications

• Basic Life Support (BLS) certification required prior to start.

  • Acceptable certifications: AHA, Red Cross, CHSF, or Military.
  • Preference for AHA certification.

Licenses

• Primary source verification of professional license/certification required within 30 days of start.
• Massachusetts Conditional Licenses are not accepted.

Professional Requirements

• Nurse Aide Registry check (within 30 days of start).
• Criminal background check within 45 days prior to start, including county and international searches if applicable.
• Massachusetts CORI background check within 30 days prior to start.
• Resume including past 7 years of work experience with city and state.
• Two recent references (minimum one supervisor or above).
• Skills checklist.
• Confirmation of assignment/POI form upon acceptance.
• Education verification of highest level achieved.
• Social Security Number trace with criminal check.
• National sex offender registry check within 30 days of start.
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually.

Personal Health History Requirements

• Documentation of MMR vaccination or proof of immunity.
• Influenza vaccine required during flu season (Sept 15 - April 30); no exemptions allowed.
• TB test documentation within 3 months of start (skin test or blood test) and updated if >6 month break.
• Physician health statement within 365 days of start and updated if >6 month break.
• Respirator Medical Clearance attestation if fit testing is required.
• Hepatitis B vaccination or immunity strongly preferred.
• Tdap vaccine within 10 years preferred.
• Positive PPD requires negative chest x-ray documentation (optional but recommended).
• TB symptom survey if positive PPD (optional but recommended).

• All required documents and verifications must be completed and uploaded as specified prior to or upon onboarding.
• Flu vaccine documentation must be uploaded as a PDF.
• No religious or medical exemptions for flu vaccine accepted.
• Background and credential checks are subject to final approval by the healthcare system’s security and compliance teams.
