Physical Therapist Outpatient - Orleans, Massachusetts

Job Summary

• Assist patients in reaching maximum physical performance while adjusting to disabilities.
• Restore patient physical functioning by interpreting physical therapy evaluations and test results.
• Determine physical therapy treatment goals and plans.
• Administer manual exercises.
• Instruct, encourage, and assist patients in performing physical activities and using assistive/supportive devices.

Required Credentials and Compliance

Certifications

• BLS (Basic Life Support): Required for onboarding. Acceptable certifications include AHA, Red Cross, CHSF, or Military. Preference for AHA certification.

Licenses

• Professional License Verification: Must provide primary source verification of professional license/certification within 30 days of start date. MA Conditional Licenses are not accepted.

Professional Requirements

• Nurse Aide Registry Check: Required within 30 days of start (applies to all employees per MA DPH).
• Orientation: Completion of Spaulding Orientation Summary.
• Criminal Background Check: Within 45 days prior to start date, including county and international searches if applicable.
• Massachusetts CORI Check: Completed within 30 days prior to start.
• Resume: Must include past 7 years of work experience with city and state.
• References: Two recent references required, at least one supervisor or above.
• Skills Checklist: Required for submission.
• Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
• Education Verification: Verified through background check company.
• SSN Trace: Must be run with criminal check.
• National Sex Offender Check: Ran within 30 days of start and updated annually.
• Exclusion Checks: Ran within 30 days of start and annually (OIG, GSA, MassHealth, NH Medicaid).

Personal Health History Requirements

• MMR Vaccination or Immunity Proof: Documentation of measles, mumps, and rubella vaccines or immunity via blood test.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• TB Test: Documentation of TB skin test or negative blood test within 3 months of start date; updated if >6 month break.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break.
• Respirator Medical Clearance (if applicable): Attestation required if fit test is needed.
• Optional but Preferred: Hepatitis B vaccination or immunity proof; Tdap vaccine within 10 years.
• Positive PPD: If positive TB test, documentation of negative chest x-ray required.

This position requires compliance with all credentialing, background checks, and health requirements prior to onboarding.
