Registered Nurse Travel/Contract Position

Location: Mayfield Heights, Ohio
Contract Length: 13 weeks

Requirements & Compliance

Identification & Background

  • Government-issued photo ID (Driver's License or State ID) required and must be carried on assignment.
  • Comprehensive background check required prior to start, including:
    • FCRA compliant 7-year search (SSN Trace, Counties, States, Nationwide)
    • National Sex Offender Registry
    • OFAC, FACIS Level 3
    • Education verification (international verifications require CGFNS documentation as placeholder if delayed)
  • Background checks must be updated annually and after breaks in service of 2+ months.
  • Monthly EPLS/GSA/SAM and HHS/OIG checks required.
  • Fingerprinting (BCI and FBI) must be completed within 48 hours of signed contract and results updated annually.
    • Fingerprints done under code 4723.09 (Nursing).
    • Facility will also conduct their own fingerprinting onsite.
    • Candidates may start with fingerprint results pending if fingerprinting is completed prior to start.
    • Fingerprint results must be received before extension contract start.

Licensure & Certifications

  • State nursing license primary source verification required within 30 days of start for new and extension contracts.
  • NURSYS verification required within 30 days of start.
  • Required certifications (must be current or renewed if expiring within 30 days of start):
    • BLS (Basic Life Support) – required for all candidates
    • ACLS (Advanced Cardiac Life Support) – required for many specialty areas (e.g., ICU, Med/Surg, Dialysis, Oncology, etc.)
    • PALS (Pediatric Advanced Life Support) – required for PICU, Pediatric Stepdown, ERPediatric OR; preferred for PACU
    • NRP (Neonatal Resuscitation Program) – required for Labor & Delivery, OB/GYN, NICU
    • TNCC (Trauma Nursing Core Course) – preferred but not required for ER
    • Chemotherapy certification – not required for Oncology
    • Tech Cert Verification – required for CVOR and NBSTSA certifications
  • Annual acknowledgment of Orientation Safety Manual and Pass/Fail Testing (EKG/Medication) required for most nursing positions (except OR).
  • Unit-specific competency exams required within 12 months of start and updated annually.

Health & Immunization

  • Physical exam within 1 year of start, including assessment of ability to perform essential job functions and Snellen eye exam (for candidates signing 4/11/2025 or later).
  • Fit mask (N95) fit testing required within 12 months of start and updated annually. PAPRs approved on a case-by-case basis.
  • Immunizations and documentation required:
    • Tetanus/Diphtheria/Tdap (every 10 years)
    • Negative TB test (2-step PPD or QGold/TSPOT) or chest X-ray if history of positive TB, updated annually
    • MMR (Measles, Mumps, Rubella) – 2 vaccines or positive titer; booster required if titers negative/equivocal
    • Hepatitis B – titer, series, or declination documentation
    • Varicella – 2 vaccines or positive titer; booster required if titers negative/equivocal
    • Flu vaccine or approved medical/religious exemption required during flu season (10/31 - 03/31)
  • Drug screening: Standard 10-panel drug screen within 12 months of start, updated annually (instant drug screens not accepted).
  • Eye exam required within 1 year of start for new candidates signing 4/11/2025 or later.

Training & Orientation

  • Completion of facility-specific online training modules prior to start is mandatory.
    • Modules must be completed on a computer using Google Chrome.
    • Training videos must be watched before tests; skipping videos locks candidate out.
    • Modules typically take multiple days to complete and are high priority.
  • Pass/Fail EKG and Medication testing completed onsite during orientation for bedside RNs (except OR).
  • SafeMedicate preparation materials provided for review before orientation testing.
  • Candidates returning to the system with less than 6 months gap may have some orientation/testing exemptions.

Employment History & References

  • Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care and similar area to assignment.
  • Two professional references required from employment within 1 year of start, from a manager, supervisor, or charge nurse (co-worker references not accepted).
