Physical Therapist - Travel/Contract Position

Location: Roseburg, Oregon
Specialty: Therapy/Rehabilitation (Physical Therapy)
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide Physical Therapy services in a Home Health setting with home visits and periodic office meetings.
  • Average patients per day: 5-6
  • Patient load: 5-10 patients per day, equating to roughly 6-8 points daily (30 points per week expected).
  • Types of cases: Start of Care and Follow Ups.
  • Schedule: Monday to Friday, 8 AM to 5 PM. No weekend rotation or on-call required.
  • Must be willing to travel up to 75 miles from the home clinic, averaging 100+ miles per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
  • EMR systems used: Point Click Care / HCHB.
  • Position created due to full-time PT dropping to PRN status.
  • Local candidates accepted at the same rate.

Requirements

  • Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted).
  • Current Physical Therapy license required (pending licenses accepted only if license will be obtained within a few days).
  • Certifications: CPR and BLS required (AHA, National CPR Foundation, or ARC accepted).
  • Reliable transportation and current driver's license required.
  • Agency-required liability insurance required.
  • Willingness to discuss weekend and on-call requirements during interview as they may vary by location.

Compliance & Documentation

Candidates must complete and maintain the following prior to start and throughout the assignment as applicable:

Background Checks & Verifications

  • Standard 7-year background check including SS trace, county, nationwide, sex offender, and state-specific searches.
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
  • Fingerprinting as required by state (varies by location).
  • Education verification (degree applicable to profession).
  • Work history verification (at least 2 most recent employers or last 7 years).
  • References: Two required for all placements.

Health & Immunizations

  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
  • 10-panel drug screen.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination series or documented declination.

Facility-Specific Documents & Training

  • Personal or auto insurance policy (must be current and under candidate’s name or list candidate as driver).
  • Review and sign various compliance documents including:
    • NDA and End User Agreement
    • Compliance Basics and Practice Quiz
    • Code of Conduct
    • HIPAA and Privacy Policy
    • Confidentiality Form and Covenant
    • Home Health and Hospice Core Policy Acknowledgements and Scenarios (as applicable)
    • Employee Responsibility Agreement
    • COVID-19 Policy and Attestation
    • TB Questionnaire (and TB Risk Assessment if positive)
    • Other state-specific forms and attestations as required.

Additional Notes

  • Candidates must sign new supplier packets at the start of a new assignment and annually if working more than one consecutive year.
  • Some positions may follow a Baylor Shift schedule (4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
  • On-call and weekend requirements vary by location and should be discussed during the interview.

This position is ideal for experienced Physical Therapists with recent home health experience who are comfortable with travel and working independently in a home health environment.
