Job Opportunity: Physical Therapist - Outpatient (Travel/Contract)

Location: Decatur, Illinois

Job Details

  • Shift Options:
    • 4x10 hour shifts (8:00 AM - 6:30 PM)
    • 5x8 hour shifts (8:00 AM - 4:30 PM)
  • Years of Experience Required: Minimum 1 year
  • First-time Travelers: Accepted
  • Weekend Requirement: None (unless desired)
  • On-call Requirement: No
  • Floating: Clinician may float within scope as needed
  • Block Scheduling: Open to discussion with manager
  • Local Candidates: Accepted (no radius restrictions)
  • Return to Office Restrictions: Ideally no more than 5 days
  • Guaranteed Hours: Client may call off up to 36 hours per 13-week assignment without pay; additional cancellations may be billed

Required Certifications & Licenses

  • BLS (Basic Life Support)
  • State Physical Therapy License

Additional certifications such as ACLS or PALS may be required based on unit/specialty.


Experience & Requirements

  • Minimum 1 year of experience in Physical Therapy (Outpatient)
  • Prior travel experience: Not required
  • Must be able to float within scope of practice
  • Must be able to accommodate flexible scheduling requests

Compliance & Documentation Requirements

  • Valid/current US Driver’s License or State ID
  • Background checks covering all locations lived/worked in past 7 years, updated annually
  • EPLS/GSA/SAM, HHS/OIG checks within 30 days prior to start
  • Illinois-specific background checks including Department of Corrections, Most Wanted, and State Police registries within 30 days prior to start
  • Fingerprinting through Illinois Health Care Worker Registry (for clinical candidates without IL state license) completed in Illinois 3 business days prior to start
  • Education verification (degree/transcript/diploma)
  • Competency exam (required for all clinical candidates)
  • Photo for badge (color photo, mid-shoulders up, plain background)
  • Signed Code of Conduct
  • Flu vaccine or approved medical/religious exemption required annually
  • Physical exam within 1 year of start, signed by appropriate healthcare provider, confirming ability to perform job functions
  • Fit mask test (for new contracts starting 09/02/25 and later)
  • Negative TB test or chest X-ray within 1 year
  • TB questionnaire updated annually
  • Immunizations or proof of immunity for Measles, Mumps, Rubella, Hepatitis B, Varicella
  • 9-panel drug screen within 30 days of start
  • References: 2 professional references required for clinical candidates
  • Skills checklist required for clinical candidates

  • Non-clinical or remote candidates may have reduced documentation requirements.
  • Facility accommodates latex allergies.
  • Parking permit form and badge name confirmation required within 7 days of contract signing.
