Health and Immunization Requirements
- MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
- Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire (updated annually)
- 2 Step PPD OR IGRA (Quantiferon/Tspot) within required timeframes
- Varicella: Proof of immunity or two doses of vaccine (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Fit Test: Required annually (preferred prior to start)
Professional and Background Requirements
- Ascension Profile (resume or professional profile)
- At least two professional references prior to first assignment
- Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days of assignment, updated annually)
- Employment Verification for past seven years (updated annually)
- Education Verification relevant to clinical work (within 90 days of start)
- Ascension Confirmation of Assignment (agency letterhead documentation)
- Ascension EPLS check (within 90 days of start, rerun if break in service > 90 days)
Additional Notes
- Nursys verification and disciplinary action disclosures required
- Various onboarding attestations and checklists must be completed prior to or at start
- Physical exam or statement of good health required prior to first assignment and updated annually
- Some optional checklists may be requested by unit leadership after start
