Job Opportunity: RN Cath Lab - Iowa (Waterloo)

Position Details

  • Varicella (proof of 1 vaccination or IgG titer)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
  • Physical/Health Statement (exam within last year confirming fitness to work)
  • Tdap vaccine (current, non-expired; declination accepted if no pediatric interaction)

Professional & Background Requirements

  • Trinity Profile submission (resume, skills checklist within a year, 2 professional references)
  • Trinity Code of Conduct Form (signed)
  • Trinity I9 form (completed by agency)
  • Confidentiality and System Security Agreement (signed)
  • Integrity and Audit Services Form (signed shortly before start)
  • Non-Employee Upload Form (pages 1-3 and 28 completed)
  • Specialty Specific Competency Testing (copy of specialty exam within last year)
  • Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days of start)
  • Education Verification (highest academic degree or diploma/GED verified)
  • EPLS (Excluded Parties List System) check within 30 days
  • GSA/OIG exclusion check within 30 days
  • Joint Commission CORE Exam within last year
  • NSO (National Sex Offender) check within 30 days
