Job Opportunity: Physical Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient ages range from birth to over 100 years. Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate services. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.

Requirements

Licensure & Experience

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of Home Care experience required.
• Ability to travel to assigned cases.
• Weekend rotations occur approximately once every 6 weeks.

Prior Travel Experience

• No explicit prior travel experience required.

Credentials & Compliance (Required for Onboarding)

• Physical Therapist Professional License verification (no conditional licenses accepted).
• MVR (Motor Vehicle Record) check within 45 days of start (required for all home care positions).
• Criminal Background Check within 45 days prior to start, including Massachusetts CORI and National Sex Offender Check.
• Education verification of highest degree achieved.
• Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists.
• Fit Test for N95 respirator within 365 days of start, with Respirator Medical Clearance attestation.
• Health Statement signed by MD/DO/NP/PA within 365 days of start.
• Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed.
• TB test documentation within 3 months of start (skin test or blood test), updated if >6 month break.
• MMR vaccination or immunity proof.
• MGB Nurse Aide Registry check within 30 days of start (required by MA DPH for all employees).
• Additional health requirements: Hepatitis B (strongly preferred), Tdap (preferred), Positive PPD with negative chest x-ray if applicable.

Submission Requirements (Prior to Onboarding)

• Resume including past 7 years of work experience with city and state.
• Two recent references (minimum one supervisor or above in skillset).
• Skills checklist.
• Candidate submission coversheet.
• MGB Badge Photo (color photo taken within past 6 months, no selfies or filters).

Additional Information

• Weekend rotations approximately once every 6 weeks.
• Must be able to travel to patient homes as assigned.
