Job Description

• Provide Physical Therapy services in Home Health settings, including home visits and periodic office meetings.
• Average patients per day: 5-6.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts Friday-Monday).
• Patient visits may occur in patients' homes and care facilities wherever Home Health can be safely provided.
• Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate (compensated from patient home to patient home; commute to/from first and last patient is not billable).
• Local candidates accepted at the same rate.
• On-call requirement: At least once per month (varies by location).
• Weekend requirements vary by location and profession; discuss during interview.

Requirements

• Specialty: Physical Therapist in Home Health.
• Experience: Minimum 1 full year of recent Adult Home Health experience required.
• First-timers: Not accepted; must have recent Home Health experience.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have license in hand; pending licenses accepted only if expected within a few days.
• Transportation: Reliable transportation and current driver's license required.
• Insurance: Agency-required liability insurance required.
• EMR Systems: HomeCare HomeBase, Oasis, Outlook.

Productivity Expectations

• Weekly productivity expectation: 30 units.
• Patient load: Contractor assigned between 5-10 patients per day, equating to roughly 6-8 points per day.

Additional Information

• Guaranteed hours: No. If unable to meet financial needs due to low patient census, floating to a nearby clinic may be discussed.
• Hiring option: Possibility to be hired at the end of the contract if a good fit.
• Must be willing to comply with all required background checks, authorizations, and facility-specific documentation.
• Must review and sign compliance, confidentiality, privacy, and other policy documents prior to start.
• Required health documents include physical exam, negative TB test or chest X-ray, immunizations (Measles, Mumps, Rubella, Hepatitis B, Varicella), drug testing, flu vaccination or declination, and COVID vaccination or declination.
• References: Two references required.
• Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.
