Certifications & Licenses Required
- Basic Life Support (BLS): Required for all candidates (AHA only; RQI acceptable but non-RQI preferred). Not required for Histology Techs or MLTs.
- Advanced Cardiac Life Support (ACLS): Required for many units including Endoscopy, ER, ICU, MICU, SICU, CVICU, Neuro ICU, Med/Surg, TCU, PACU, PICU, Pediatric PCU, Pediatric Stepdown, L&D, OB/GYN, Clinical Decisions, Tele, Oncology, Apheresis, Cath lab, Stepdown Neuro and Step Down. Preferred but not required for dialysis. Not required for CVOR, OR, Pediatric OR, Ambulatory Care & Behavioral Health.
- Pediatric Advanced Life Support (PALS): Required for PICU & Pediatric Stepdown. Preferred but not required for PACU. Not required for ER or Pediatric OR.
- Neonatal Resuscitation Program (NRP): Required for L&D, OB/GYN, and NICU.
- National Tech Certifications: Required for CVOR, SACST, Histology Tech (ASCP preferred), Polysomnographer/Sleep Tech (BRPT required).
- NIH Stroke Certification: Required for PCU, Stepdown Neuro, ICU, MICU, CVICU, SICU, Neuro ICU, Clinical Decisions, ER, Observation, Tele, MS/Tele, TCU, Step Down, SNF units. Preferred but not required for PICU, Pediatric Stepdown, & Pediatric PCU.
- Other Certifications:
- TNCC (preferred but not required for ER)
- Chemotherapy certification (not required for oncology)
- Orientation Safety Manual Acknowledgement (HIPAA)
- Pass/Fail Testing (EKG/Medication) required for bedside RNs (not required for OR RNs)
Background & Compliance Checks
- Criminal background check required prior to start; must be updated annually and after breaks in service of 2+ months.
- Monthly EPLS/GSA/SAM and HHS/OIG checks required.
- Fingerprinting (FBI and BCI) must be completed within 48 hours of contract signing and updated annually. Fingerprints must be under code 4723.09 (Nursing). Candidates can start with results pending if fingerprinting is completed prior to start. Not required for Histology Techs.
- Education verification required for all new candidates.
- State license primary source verification required within 30 days of start for new and extension contracts. License must be current and renewed if expiring within 30 days of start.
- NURSYS verification required within 30 days of start for RN, LPN/LVN, AP.
Health & Immunization Requirements
- Physical exam within 1 year of start, including assessment of ability to perform essential job functions. Latex allergy notification required.
- Fit mask test (3M N95 1860 small/regular or 1870+) within 12 months of start and updated annually. Not required for Histology Techs.
- Tdap vaccine every 10 years (official immunization record required). Religious/medical exemptions reviewed case-by-case; declinations not accepted.
- Negative TB test or chest x-ray within 12 months for RN/LPN/CSFA/CST/CVOR Tech candidates; within 6 months for Xray Techs, MLT, CT Tech, EEG Techs, Sleep Techs, Cath lab Techs, Histology Techs, Ultrasound Tech, Phlebotomist, Interventional Radiology, RRT. Annual updates required.
- TB questionnaire required annually if history of positive TB test.
- Immunizations or titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, vaccine series, or signed declination)
- Varicella
- Flu vaccine or approved medical/religious exemption required during flu season (10/31 - 03/31). Declinations not accepted.
Documentation & Onboarding Requirements
- Two satisfactory pre-employment references within 1 year of start required from a manager, supervisor, charge nurse, or director (no co-worker references).
- Employment application must include 3 years prior work history with at least 2 years clinical practice in acute care in a similar area.
- Unit-specific skills checklist required, updated annually.
- Completion of facility-specific onboarding modules prior to start is mandatory. Modules must be completed on a computer using Google Chrome; video training must be watched before testing.
- Candidates must pick up their badge from the facility prior to start.
- Various facility-specific checklists and forms must be completed and brought to orientation (e.g., compliance checklist, contract personnel checklist, patient care checklist, unit checklist, BMV/eMAR acknowledgement, IS access form, COVID-19 questionnaire, confidentiality agreement, orientation packet).
Additional Notes
- Candidates with a break in employment of less than or equal to 40 days may not need to repeat physical exam or chest x-ray if current documentation is valid.
- Fingerprinting results must be received prior to extension contracts; exceptions require facility approval.
- Candidates must carry a valid government-issued photo ID while on assignment.
- Pass/fail medication and EKG testing required onsite during orientation for bedside RNs. Retakes may be available after 1 week if failed.
This position requires strict adherence to all certification, health, background, and onboarding requirements to ensure compliance with facility and state regulations.
