Therapy PT Home Health Travel/Contract Job - Marlborough, Massachusetts

Job Details

• Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend Rotation: No weekend rotation required

• Patient Load: 5-10 patients per day (approximately 6-8 points daily, 30 points per week)
• Types of Cases: Start of Care, Follow Ups
• Territory Coverage: Must be willing to travel up to 75 miles from home clinic; weekly travel estimated between 50-200 miles
• EMR System: HCHB (Homecare Homebase)

Experience & Requirements

• Years of Experience Required: Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted)
• Certifications Required: CPR and BLS (Basic Life Support) current certification
• Licensure: Must have license in hand (pending licenses accepted only if expected within a few days)
• Local Candidates: Accepted at the same rate
• Transportation: Must have reliable transportation and a current driver's license
• Mileage Reimbursement: Paid from patient home to patient home (commute to/from first and last patient not billable)
• Weekend and On-Call Requirements: Vary by location; discuss during interview

Additional Requirements & Compliance

• Must complete all required background checks, including:

  • 7-year standard background check (including SS trace, county, nationwide, sex offender, state-specific searches)
  • EPLS/GSA/SAM and HHS/OIG screenings
  • Fingerprinting as required by state (varies by location)
• Must complete education verification
• Must provide two professional references
• Must verify work history for at least the last 7 years or two most recent employers
• Must complete all required state-specific and facility-specific documents and attestations prior to start
• Must comply with Massachusetts state-mandated background checks (MA CORI)
• Must complete health requirements including:

  • Physical exam within 12 months prior to start
  • Negative TB test or chest X-ray within 12 months, updated annually
  • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
  • 10-panel drug screen
  • Seasonal flu vaccination or documented declination
  • COVID-19 vaccination per Massachusetts state policy (full vaccination required, no declinations accepted)
• Must review and sign facility-specific compliance documents including:

  • Code of Conduct
  • Confidentiality forms
  • HIPAA and Privacy Policy
  • Compliance training and quizzes
  • Home Health Scenarios and Core Policy Acknowledgements
  • Employee Responsibility Agreement
  • COVID-19 Policy and Attestation
• Must have agency-required liability insurance and personal/auto insurance (policy must be current and under candidate’s name or list candidate as driver)
