Job Opportunity: Ultrasound Technician

Location: Middleburg, Florida
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule & Requirements

• 40 hours per week
• Every weekend required
• Easter availability required

Credentials & Certifications Required

• Required:

  • ARDMS General
  • BLS (American Heart Association)
  • Current state license with primary source verification (within 30 days prior to start)
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly)
  • Ascension Medical Imaging Contract Worker Onboarding Education Attestation
  • MMR (proof of two vaccines or positive IGG titer; no declinations accepted)
  • Fit Test (updated annually)
  • Hepatitis B vaccination series or signed declination
  • Influenza Vaccine (seasonal; exemptions accepted with mask requirement)
  • Health Statement or physical exam (completed prior to first assignment, updated annually)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) testing (within required timeframes)
  • Varicella (proof of immunity or two vaccines; no declinations accepted)
  • Tdap (valid within 10 years; no declinations accepted)
  • Ascension Skills Checklist (clinical, completed within one year of start, updated annually)
  • Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
  • Ascension Education Verification (within 90 days of start, updated if break in service > 90 days)
  • Ascension Confirmation of Assignment (provided upon initial assignment and updates)
  • Ascension Employment Verification (past 7 years, updated annually)
  • Ascension Criminal Background Check (including SSN Trace, OIG Search, National Sex Offender; updated annually)
  • Ascension EPLS (within 90 days of start, rerun if break in service > 90 days)
  • Ascension Profile (resume or professional profile)
  • Ascension 2 Professional References (at least two professional references)
  • Driver's License/State ID (must be current and valid)
  • Ascension Badge Photo (headshot with white or blue background, no alterations)

• Preferred:

  • Vascular credential
