• Administration of respiratory therapies throughout the facility including ICU, NICU, and the emergency department.

Required Credentials & Compliance

Certifications & Licenses

• BLS (AHA only) – Required for onboarding
• State License Verification – Must be completed within 30 days prior to start, verified by state board
• License/Certificate Verification – Completed within 30 days prior to start
• Driver's License/State ID – Valid, current US driver’s license or ID (cannot be expired)

Professional Requirements

• Arnot Skills Checklist – Must be fully complete and updated annually for the unit assigned
• Arnot Core Competency Exam – Must score 80% or higher; includes Core Comp I, II, III depending on role
• Arnot Resume – Must account for all employment and gaps for the last 7 years, with education history
• Arnot 2 Professional References – One supervisor reference from last 12 months and one supervisor or peer reference from last 3 years
• Arnot Job Description – Signed and dated
• Arnot Employment Verification – Verification of most recent employer required
• Agency Background Check – Must be run within 30 days of start, including SSN address trace, county and national criminal checks, FACIS 3 check
• Agency Drug Screen – Must be run within 30 days of start; specific drug panel required (marijuana testing not permitted)
• 7 Instant Background Checks – Includes multiple federal and state exclusion and registry checks
• Arnot Health Personal Information Form, Code of Conduct, Confirmation of Assignment Form, Self Study Packet Documentation and Quiz, Sexual Harassment Acknowledgement, Parking Registration Form – All required and found in Document Library
• AH Drug Tobacco Acknowledgement – Required

Personal Health History & Immunizations

• COVID-19 Vaccine – Optional, but proof or declination can be uploaded
• MMR – Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series required within 30 days of start
• Fit Test – Must be completed prior to start and within last 12 months; specific approved mask models required
• Hepatitis B – Positive antibody titer, 3 vaccine series, or specific declination form required
• Influenza Vaccine – Required or specific declination form; if declined, mask must be worn at all times on assignment
• TB Test – One of the following required: 2 skin PPDs (1 within 12 months, 1 within 3 months) or 1 Quant/Spot within 12 months; annual requirement; follow-up steps required for past positives including chest x-ray
• Varicella – Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series required within 30 days of start
• Tdap – Preferred but not required; if no record, specific declination form required
• Physical/Health Statement – Must be from within last 12 months; includes height, weight, vitals, Snellen eye chart, and statement of no restrictions; approved providers include MD, NP, PA, DC, DO
