Job Summary: Travel Physical Therapist (Home Health)
Location: Lakeport, California
Specialty: Therapy/Rehabilitation (Physical Therapy)

Contract Length: 13 weeks
Requirements
- Valid California Physical Therapist (PT) License
- At least 1 year of experience as a Home Health Physical Therapist
- Valid California driver's license and auto insurance
- BLS certification preferred (not required)
Background Checks & Compliance
- 7-year background check required within 6 months of start, including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, motor vehicle report (for driving positions), and FACIS Level 3 screening within 2 weeks of start.
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
- Sex offender search must be run within 90 days of start.
- Education verification required unless highest education is high school.
- Transcript/diploma verification as needed, including international education.
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
- Employment application must include full 7 years of work history with no gaps.
- Employment verification for up to 7 years required; gaps over 30 days must be explained.
Certifications
- State License: Copy and primary source online verification required within 30 days of start.
- BLS (Basic Life Support) preferred.
- Other certifications (ACLS, PALS, NRP, CCRN, TNCC, EFM, CPI, NIH Stroke Certification) to be verified as applicable by position.
- Tech Certification verification required within 30 days of start as applicable.
Health & Immunization Requirements
- Physical exam within 1 year of start (must indicate cleared for work or specify work limitations).
- Fit mask test within 1 year of start, renewed annually (exceptions apply).
- Immunizations and/or titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test or chest x-ray within 90 days of start, renewed annually
- Measles (Rubeola), Mumps, Rubella: Positive titer or vaccination series with boosters as needed
- Hepatitis B: Positive titer, series, or signed declination
- Varicella: Positive titer or vaccination series with boosters as needed
- Seasonal flu vaccination required during flu season (11/1 - 4/30), with signed declination accepted.
- Drug testing: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids).
Facility Specific Documents & Training
- Candidates must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and regulatory orientation prior to start.
- Religious exemption forms required for PAPR use and vaccine exemptions.
Additional Notes
- Prior travel experience is not explicitly required for this position.
- Driving position: Auto insurance and valid driver's license required.
- References: 2 professional references required for clinical/allied/per diem acute care candidates; 1 for per diem non-acute care (Aya requirement only).
