Job Description

Required Certifications & Licenses

• BLS (AHA certification required)
• Current RN license with primary source verification within 30 days prior to assignment
• Nursys verification within 30 days prior to start
• Ascension-specific competencies and exams (e.g., Core Competencies, EKG Exam)
• Driver’s License/State ID (must be valid and match required address criteria)

Required Health & Immunizations

• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Varicella: Proof of immunity or two doses of vaccine (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• TB Screening: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire
• Fit Test: Required annually (preferred prior to start)
• 12 Panel Drug Screen: Must include all 12 drugs tested with MRO review if applicable

Additional Requirements

• Ascension onboarding and education attestations
• Skills checklist and clinical competency verification within one year, updated annually
• Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment
• Employment and education verifications within 90 days of start, updated annually or after breaks in service
• Professional references: At least two completed prior to first assignment
• Resume/Profile submission required
• Post-assignment evaluations may be requested
