Job Opportunity: RN Medical/Surgical
Location: Evansville, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Unit & Patient Population
- Pulmonary & Medical Overflow patients
- 22 beds
- Patient Ratio: 1:4-5
Required Specialty Experience
- RN Medical/Surgical (Years of experience not explicitly stated; assume standard for specialty)
Certifications & Licenses Required
- BLS: Must be American Heart Association (AHA) certified
- Nursys Verification: Required within 30 days prior to start
- Current Nursing License: Primary source verification within 30 days prior to start
- Driver's License/State ID: Must be valid and match address requirements
Testing & Exams Required Prior to Start and Ongoing
- Ascension Pharmacology Exam (minimum 80%, 2 attempts max, updated every 3 years)
- Ascension Medical Surgical Exam (minimum 80%, 2 attempts max, updated every 3 years)
- Ascension EKG Exam (minimum 80%, 2 attempts max, updated as new editions released)
- Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, 2 attempts max, updated yearly)
- Ascension Skills Checklist (clinical specialty-specific; completed within 1 year of start, updated annually)
- Ascension Unit Specific Checklist (optional for onboarding)
Health & Immunization Requirements
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
- Varicella: Documented proof of two vaccines, serological immunity, or verified history; no declinations accepted
- Tdap: Valid within 10 years; no declinations accepted
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
- Fit Test: Required for all travelers; preferred before start, can be completed onsite
- TB Testing: 2 Step PPD or IGRA within required timeframes; positive results require chest X-ray and annual questionnaire
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
- 12 Panel Drug Screen: Within 30 days prior to start, updated annually; includes 12 specified substances
Professional Requirements
- Resume/Profile submission required
- At least two professional references required prior to first assignment
- Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days of start, updated annually)
- EPLS check within 90 days of start, rerun if break in service > 90 days
- Education Verification relevant to clinical work within 90 days of start, rerun if break in service > 90 days
- Employment Verification for past 7 years, updated annually
- Completion of Ascension Nursing Contract Worker Onboarding (done onsite)
- Completion of Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
- Ascension Confirmation of Assignment documentation upon initial assignment and extensions
- Badge Photo: Forward-facing headshot with white or blue background, no alterations
