Job Description

• Work in a Level III NICU/PICU setting providing respiratory therapy services
• Night shift position

Required Certifications and Licenses

• Current Respiratory Therapist license with primary source verification within 30 days prior to assignment start
• BLS (American Heart Association)
• PALS (American Heart Association)
• ACLS (American Heart Association)
• NRP (Neonatal Resuscitation Program) - AHA/American Academy of Pediatrics
• Driver’s License/State ID (valid, must match address requirements)

Health and Safety Requirements

• Documentation of two MMR vaccines or positive IGG titer (no declinations accepted)
• Fit Test (PAPR not completed onsite; specific mask types required)
• Hepatitis B vaccination series completion or signed declination
• Influenza vaccine (seasonal; exemptions accepted with mask requirement)
• Positive IGRA or PPD results with negative chest X-ray within one year, plus annual TB questionnaire
• Two-step PPD or IGRA within specified timeframes
• Varicella vaccination or immunity documentation (no declinations accepted)
• Tdap vaccination valid within 10 years (no declinations accepted)
• Health statement or physical exam prior to first assignment, updated annually
• 12 Panel Drug Screen within 30 days prior to start, updated annually

Onboarding and Compliance Requirements

• Completion of Ascension Respiratory Therapy Contract Worker Onboarding and Education Attestation (6 hours pre-hire training)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) completed within one year, updated yearly
• Ascension Skills Checklist (clinical competency) completed within one year, updated annually
• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start
• EPLS (Excluded Parties List System) check within 90 days of start
• Education verification relevant to clinical work within 90 days of start
• Employment verification for the past seven years
• Confirmation of assignment documentation on agency letterhead
• Ascension Badge Photo (headshot with white or blue background, no alterations)
