Job Opportunity: PT Home Health

Location: Saint Joseph, Missouri

Job Details

• Shift: 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
• Call: Required 1 Sunday every 5 weeks
• Weekend Schedule: Every 5th week, take 2 days off during the week and work Saturday and Sunday to perform admissions
• Counties Served: Andrew, Atchison, Buchanan, Clay, Clinton, Daviess, Dekalb, Gentry, Harrison, Holt, Platte, and Worth
• Travel: Covers 13 counties; travel to furthest areas rotates weekly. On high mileage/travel days, fewer patients are scheduled.
• Dress Code: No printed scrubs; black, grey, or khaki pants with polo or scrub top

Requirements

• Experience: Minimum 1 year of experience required, must have Home Health experience
• Certifications:

  • BLS (Basic Life Support)
  • DPT (Doctor of Physical Therapy)
  • State License (Missouri)
• Prior Travel Experience: Not explicitly required

• Background Checks:

  • National Criminal Check
  • State and County Searches (10 years of all addresses and aliases)
  • SSN Trace with all aliases

  • EPLS/GSA/SAM

  • Education Verification (official transcript or diploma with graduation date)
  • Missouri EDL Search within 30 days of start

• Health Requirements:

  • Physical Exam (within last 12 months)
  • Fit Mask testing completed onsite
  • Negative TB test or Chest X-Ray within 1 year, updated annually
  • Immunizations or proof/declination for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug Screen within 30 days of start (includes multiple drug panels)
  • Flu Vaccine required during flu season or approved medical/religious exemption (mask required if exempt)

• References & History:

  • 2 professional references within 3 years
  • Employment application and verification of most recent employer
  • Skills checklist

• Locals accepted at the same rate
• Various competency exams and facility-specific documents required prior to or at start
• Annual acknowledgements and safety manual reviews required
• Basic Dysrhythmia exam and medication competency exams may be required depending on specialty/unit (not specifically for this PT role)
