Job Opportunity: Neuro Surgical Tech (Surgical Technician)

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks

Work Schedule

• 8-hour shifts, typically 6:30 AM to 3:00 PM (may vary by department needs)
• Mostly day shifts
• Every third weekend coverage required
• Call requirement: 10-12 days per month
• Holiday rotation as needed by department

Specialty & Experience Requirements

• Specialty: Surgical Technician with Neurosurgery experience

• NBSTSA Certification

Health & Safety Requirements

• COVID-19 Vaccine: Proof of September 2025 booster, declination or exemption with agency letterhead (declination not accepted for CA or IL)
• MMR vaccination proof or IgG titer lab report
• Fit Test Mask (Iowa requires 3M 1860 mask or equivalent)
• Hepatitis B vaccination series (proof required, titers not accepted)
• Influenza vaccine for current flu season or declination with agency letterhead (declination not accepted for CA or IL)
• Varicella vaccination proof or IgG titer lab report
• TB test: 2-step PPD or IGRA (Quantiferon/Tspot) within required timeframe
• Tdap vaccine (current, declinations accepted if no pediatric interaction)
• Physical/Health Statement confirming fitness to work (exam within last year)
• 9 or 10 Panel Drug Screen completed within 30 days of start (no THC)
• Color Vision test

• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County, last 7 years) including SSN verification
• Education Verification (highest level of academic education)
• EPLS (Excluded Parties List System) check within 30 days
• OIG/GSA Exclusion check within 30 days
• Integrity and Audit Services Form signed prior to start
• Joint Commission Testing (CORE Exam within last year)
• National Sex Offender Registry check within 30 days
• System Access Form completed and uploaded one week before start
• Skills Checklist provided by agency for clinical contractors
• Child and Adult Dependent Abuse Training (Iowa only)
• Code of Conduct Form signed
• I9 verification completed by agency
• Iowa-specific Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days
• OFAC Sanctions check within 30 days
• Non-Employee Upload Form pages 1-3 and 28 completed and uploaded
