Job Description

• Evaluate and treat patients of all ages, modifying plans of care as appropriate.
• Serve as a role model and clinical resource for staff and students.
• Perform patient transfers with assistance as needed (bed, chair, floor).
• Use safe and appropriate body mechanics to reduce injury risk.
• Treat patients and colleagues with dignity, empathy, and respect.
• Design and adjust plans of care integrating goals, treatment, outcomes, and discharge plans.
• Educate patients, families, caregivers, students, and healthcare providers using effective teaching methods.
• Evaluate clinical findings to determine physical therapy diagnoses and outcomes.
• Perform examinations using reliable and valid methods relevant to patient complaints and history.
• Produce documentation in accordance with clinical policies.
• Screen patients to determine the need and effectiveness of therapy services.
• Use and monitor support personnel according to legal and ethical guidelines, providing regular feedback.

Department-Specific Functions

• Implement intervention plans using functional activities considering patient/family emotional and functional needs.
• Modify approaches based on patient behavior and performance.
• Utilize standardized outcome measures uniformly to reflect functional performance accurately.
• Complete documentation per regulatory and organizational standards.
• Collaborate with patients, families, and significant others throughout rehabilitation, providing education and family training as needed.
• Supervise PTAs and rehabilitation technicians in compliance with regulatory agencies.
• Communicate patient performance, status changes, transfer recommendations, discharge disposition, and equipment needs clearly with the medical team.

Contract Length

• 13 weeks

Required Qualifications

• Valid Physical Therapist license in Illinois
• Minimum of 1 year of Physical Therapist experience within the last year
• Bachelor's Degree in Physical Therapy
• BLS certification issued by the American Heart Association
