Job Opportunity: Physical Therapist - Therapy/Rehabilitation (Travel/Contract)
Location: Largo, Maryland
Contract Length: 13 weeks (approximately 13 weeks)

Average Travel: About 200 miles per week, willing to travel up to 75 miles from home clinic
Job Description
- Provide Physical Therapy services in a Home Health setting with home visits and periodic office meetings.
- Average patients per day: 5-6 (assigned between 5-10 patients per day equating to roughly 6-8 points).
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend rotation: No weekend rotation required.
- On-call: No on-call required.
- Workload driven by growth and patient census.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Mileage reimbursed at IRS standard rate (compensated from patient home to patient home; commute to/from first and last patient is not billable).
- Locals accepted at the same rate.
- Some positions may have a Baylor Shift schedule (4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
Requirements
- Experience: Minimum 1 full year of recent Adult Home Health experience (recent experience required; first-timers not accepted).
- Certifications: Current CPR and BLS certification required.
- Licensure: Must have license in hand (pending licenses accepted only if license is expected within a few days).
- Prior Travel Experience: Not explicitly required.
- Other: Must be willing to travel within the specified territory.
Productivity & Systems
- Weekly productivity expectations: Approximately 32 units.
- EMR Systems used: Pointcare, HCHB.
- Must have experience in Home Health (Hospice experience not required).
Compliance & Documentation Requirements
Candidates must complete and maintain the following prior to and during assignment:
- Valid Driver’s License/State ID.
- Background checks including 7-year history, SS trace, county searches, nationwide sex offender, state-specific searches, and motor vehicle record.
- Drug testing (10-panel).
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination.
- Various state-specific background and fingerprinting requirements depending on placement.
- Education verification.
- References: Two references required.
- Work history verification: At least two most recent employers or last 7 years.
- Facility-specific documents including compliance training, confidentiality forms, HIPAA and privacy policies, and other policy acknowledgments.
- Personal or auto insurance valid and current (policy must be under candidate’s name or list candidate as a driver).
Additional Notes
- Weekend and on-call requirements may vary by location; discuss during interview.
- Candidates must complete all required compliance and facility-specific documents prior to start.
- Candidates need to sign new supplier packets at the start of a new assignment and annually if working more than one year consecutively.
- Must be able to meet productivity expectations and maintain documentation standards throughout the contract.
